    Effects of the february 6 earthquake on the agents growing in urine cultures and antibiotic resistance rates

    OBJECTIVE: The aim of this study was to investigate the effects of the February 6 earthquake on the bacteria grown in urine cultures and antibiotic resistance of patients admitted to the urology outpatient clinic. METHODS: In this study, 11,397 urine samples requested by the urology outpatient clinic between January 2022 and De- cember 2023 were examined. We evaluated the urine cultures and antibiotic resistance profiles of the pathogens in patients’ clinic in the years 2022 and 2023 separately and post-earthquake year 2023 as two separate periods, as the first 6-month period immediately after the earthquake. The pathogens grown in urine cultures and antibiotic resistance profiles were ex- amined retrospectively. Only one of the same strains isolated in consecutive repeated growths in the urine cultures of the patients was included. RESULTS: Of the 11,397 urine cultures, 6,914 were requested in 2022 and 4,483 were requested in 2023. There was no statistical difference in the growth rate between the years (13.1–14.1%). When the demographic data of the first and last 6 months of 2023 were compared, statistical differences were found between the first and last six months of 2023 in terms of the number of patients for whom culture was requested, the number of culture positives, the reproduction rate, and the number of male and female patients. The total number of patients for whom urine culture was requested was 4483 (1471 in the first six months - 3012 in the last six months) (p=0.008) and the number of culture-positive patients was 630 (184–446) (p=0.007). The reproduction rates were found to be 12.5% and 14.8%, respectively (p=0.047). It was found that the resistance rates of Escherichia coli (E. coli) to gentamicin (GN), cefuroxime (CXM), cefixime (CFM) and ceftazidime (CAZ) groups of antibiotics were higher in the first six-month period of 2023 (p<0.05). Similarly, the re - sistance rate of Klebsiella spp. to CXM and CAZ groups of antibiotics was found to be higher in the first six-month period of 2023 (p<0.05). CONCLUSION: The results of our study show that the conditions in communal living areas after the February 6 earthquake had an increasing impact on antibiotic resistance rates and antibiotic resistance rates of certain groups of antibiotics, espe- cially the Enterobacterales group were higher in the first half of 2023.

    NEGATIVE EFFECTS OF SMOKING ON SEMEN PARAMETERS AFTER VARICOCELECTOMY

    The negative effects of varicocele and smoking on infertility are known. In our study, we aimed to evaluate the effect of smoking on the healing process after varicocelectomy. 378 male patients who have undergone varicocelectomy operation were included in the study. Patients were divided into three groups as non-smokers, patients, who smoke less than 10 cigarettes per day, and patients who smoke 10 or more cigarettes per day. Semen analysis of the patients before varicocelectomy operation and three months after varicocelectomy operation were evaluated and semen volume, sperm concentration and sperm motility were evaluated according to WHO (World Health Organizations) guidelines. Patient age changed between 16 and 52 and the mean age was 27.20±6.74. When cigarette usage was evaluated, it was determined that 52.82% of patients (n=220) were non-smokers, 13.8% of patients (n=52) smoke less than 10 cigarettes per day and 28.0% of patients (n=106) smoke 10 or more cigarettes per day. Improvement in sperm concentration and motility rates after varicocelectomy was determined to be lower in 10 or more cigarette smoking group compared to other groups. It is shown that cigarettes play a negative role in recovery process after varicocelectomy operation.

    Safe Inhalational Anesthetics in Patients With Coronary Artery Disease: Effects of Sevoflurane and Desflurane on Ischemia-Modified Albumin levels

    Introduction: Sevoflurane and desflurane are commonly used anesthetic agents with known cardiac side effects. Ischemia -modified albumin (IMA) is a useful biomarker that increases in cardiac conditions. This study investiga tes the effects of desflurane and sevoflurane on cardiac parameters and IMA levels in patients with coronary artery disease (CAD). Materials and Methods: Sixty CAD patients aged over 18 years scheduled for elective surgery were enrolled. Patients were rand omly divided into two groups: group I, desflurane (D) (n=30) and group II, sevoflurane (S) (n=30). Pre - and post-operative cardiac parameters were recorded for each patient. IMA measurement in serum was determined spectrophotometrically. Results: Postoperative IMA levels were significantly higher than preoperative IMA levels in both groups. The rate of change between post- and pre-operative IMA levels was significantly lower in group S than in group D . Postoperative heart rate (HR) and mean arterial pressure (MAP) levels were significantly lower than preoperative HR and MAP levels in both groups, whereas no significant differenc es were found between the two groups with regard to pre- and post-operative HR and MAP levels. Conclusions: Sevoflurane is safer and has fewer cardiac side effects in CAD patients than desflurane. However, there is a need for comprehensive new studies evaluating large patient groups investigating this situation.
