    Antidepresanların ilaç ilişkili hareket bozuklukları ve cinsel yan etkiler açısından karşılaştırılması

    Amaç: Çalışmamızda farklı antidepresan gruplarının cinsel yan etkiler, daha az bilinen uyku bruksizmi ve nörolojik yan etkiler açısından karşılaştırılması amaçlanmıştır. Gereç ve Yöntem: Çalışmanın örneklem grubunu 18-50 yaş aralığında, depresif bozukluklar ve/veya anksiyete bozuklukları tanısı ile ayaktan tedavi gören, 1 aydan uzun süredir antidepresan monoterapisi alan 406 hasta oluşturmaktadır. Tüm hastalara; sosyodemografik veri formu, Hamilton Depresyon Ölçeği (HAM-D), Hamilton Anksiyete Ölçeği (HAM-A), UKU Yan Etki Değerlendirme Ölçeği nörolojik yan etkiler bölümü, Arizona Cinsel Yaşantılar Ölçeği (ACYÖ) ve Morisky İlaç Uyum Ölçeği uygulanmıştır. Kullanılan antidepresanlar; seçici serotonin geri alım inhibitörleri (SSGİ), serotonin norepinefrin geri alım inhibitörleri (SNGİ ve vortioksetin olmak üzere üç gruba ayrılarak karşılaştırılmıştır. Bulgular: YAB tanılı hastalarda SNGİ kullananların UKU nörolojik yan etkiler ortalama puanı SSGİ kullananlardan anlamlı olarak yüksek belirlendi. Depresif bozukluk tanılı hastalarda; ilaç tedavisine yeterli uyum sırasıyla %91.3 oranında vortioksetin, % 73.8 oranında SSGİ ve %50 oranında SNGİ kullanan hastalarda belirlendi ve üç grup arasındaki fark istatistiksel olarak anlamlıydı. Sonuç: Antidepresan ilaç grupları bazı yan etkiler ve tedavi uyumu açısından birbirinden farklılık gösterebilmektedir. Antidepresan kaynaklı uyku bruksizmi ve hareket bozuklukları gibi daha az bilinen yan etkilerin daha iyi anlaşılması için prospektif çalışmalara ihtiyaç bulunmaktadır.

    Clinical Heterogeneity of Major Depressive Disorder: The Role of Trauma, Dissociation, and Sleep

    Background: Major depressive disorder (MDD) is a common mental disorder characterized by a wide range of symptoms and a substantial contribution to global disease burden. Our study aimed to examine the relationships between childhood trauma, sleep quality, dissociative symptoms, posttraumatic growth, and suicidality in patients diagnosed with MDD. Methods: Our sample consisted of 115 patients with MDD and 84 healthy controls. Participants were administered the Hamilton Depression Rating Scale (HDRS), Beck Scale for Suicidal Ideation (BSSI), Pittsburgh Sleep Quality Index (PSQI), Childhood Trauma Questionnaire (CTQ-33), Dissociative Experiences Scale (DES), and Posttraumatic Growth Inventory (PTGI). Group differences were analyzed, and associations among variables within the MDD group were examined using correlation and mediation analyses. Results: Compared to healthy controls, patients with MDD had significantly higher scores on all scales except the PTGI (p < 0.001 for each). Within the MDD group, individuals with a history of suicide attempts had significantly higher CTQ total scores, physical and sexual abuse subscale scores, and DES scores than those without previous attempts. Additionally, dissociative experiences had a partial mediating role in the relationship between depression severity and suicidal ideation, as well as in the relationship between childhood traumas and sleep quality. Conclusions: The findings highlight the clinical relevance of dissociative experiences and sleep disturbances in the heterogeneous presentation of MDD and their association with illness severity and suicidality. In the follow-up and treatment process of patients with MDD, risk and protective factors should be evaluated together, and individualized treatment programs should be targeted.

    Denetimli Serbestlik Polikliniğine Başvuran Madde Kullanım Bozukluğu Tanılı Bireylerde İntihar Davranışı

    Amaç: Bu çalışmada Denetimli Serbestlik (DS) Polikliniğine başvuran Madde Kullanım Bozukluğu (MKB) tanılıbireylerde psikolojik acı, çocukluk çağı ruhsal travmaları, aleksitimi, dürtüsellik ve psikolojik dayanıklılığın intihargirişimi ile ilişkisinin incelenmesi amaçlanmıştır.Yöntem: Çalışma DS Polikliniğine başvuran 120 kişi ve sağlıklı kontrol ile yapılmıştır. Tüm katılımcılara; SosyoDemografik Veri Formu, Çocukluk Çağı Ruhsal Travma Ölçeği (CTQ-28), Toronto Aleksitimi Ölçeği (TAÖ), PsikolojikAcı Ölçeği (PAÖ), Barratt Dürtüsellik ölçeği (BDÖ), Psikolojik Dayanıklılık ölçeği (PDÖ) uygulanmıştır.Bulgular: İntihar girişimi olan hastaların PAÖ, TAÖ, BDÖ, CTQ-28 ölçek puanlarının daha yüksek olduğu tespitedilmiştir. İntihar girişimi riskini tahmin etmek için kurulan modelde (forward stepwise) PAÖ, TAÖ-A, PDÖ-sosyalyeterlilik ve BDÖ-toplam değişkenlerinin modele anlamlı katkı yaptığı bulunmuştur. Anlamlı olan dört değişkeninintihar riskindeki değişimin % 65,6’sını açıkladığı tespit edilmiştir.Sonuç: MKB’lerde psikolojik acı, aleksitimi, dürtüsellik ve çocukluk çağı travmalarının yüksek, psikolojikdayanıklılığın daha düşük olmasının intihar riskini değerlendirmede öngörürücü etkenlerdir.

    Deprem Sonrası Akut Stres Bozukluğu Olanlarda Travma Esnasındaki Çözülmenin, Anksiyete Düzeyi, Algılanan Stres, Anksiyete Duyarlılığı ve Deprem Stresiyle Baş Etme ile İlişkisi

    Amaç: Çalışmamızda, deprem sonrası Akut Stres Bozukluğu (ASB) tanısı konulan hastalarda travma esnasındaki çözülmeye anksiyete duyarlılığı, anksiyete düzeyi, algılanan stres ve baş etme stratejilerinin etkisini belirlemeyi amaçladık. Yöntem: ASB tanılı 477 hastaya sosyodemografik veri formu, Beck Anksiyete Ölçeği (BAÖ), Algılanan Stres Ölçeği (ASÖ), Anksiyete duyarlılığı indeksi-3 (ADİ-3), Deprem Stresi ile Baş Etme Ölçeği ve Peritravmatik Dissosiyasyon Ölçeği (PDEQ) doldurtulmuştur. Bulgular: Travma esnasında çözülme gelişmesinde %31,5’lik değişim anksiyete duyarlılığının bilişsel alt boyutu, %7’si anksiyete, %1 ise algılanan stres ile açıklanmaktadır. Travma esnasındaki çözülme ile anksiyete arasında orta düzey pozitif yönde, algılanan stres arasında zayıf pozitif yönde, olumlu düşünme ile negatif yönde zayıf, sosyal destek arama ile negatif yönde çok zayıf ilişki bulunmuştur. Travma esnasındaki çözülme ile anksiyete duyarlığı fiziksel, bilişsel, toplumsal alt boyutları arasında pozitif yönde anlamlı orta düzey ilişki bulunmuştur. Sonuç: Araştırmamızdan elde edilen en önemli sonuç; travma esnasındaki çözülme gelişimine en fazla katkıyı anksiyete duyarlılığı bilişsel alt boyutunun yaptığının saptanmasıdır. Anksiyete duyarlılığı yüksek olan kişilerde travma esnasında çözülmenin daha fazla deneyimlenebileceği ve travmadan sonraki süreçte ilerleyen zamanlarda bu kişilerin TSSB riskinin daha fazla olacağını öngörülebilir.

    Investigation of Optic Nerve Sheath Diameter, Intraocular Pressure, and Dry Eye in Patients with Borderline Personality Disorder: The Role of Childhood Trauma

    Background/Objectives: Borderline personality disorder (BPD) is a psychiatric disorder characterized by emotional instability, impulsive behavior, and impaired interpersonal relationships. It is associated with a high prevalence of childhood trauma and neurobiological changes. This study aimed to compare ophthalmologic parameters, namely, optic nerve sheath diameter, intraocular pressure, and dry eye, in patients with BPD with healthy controls and to investigate the relations between these parameters and childhood trauma. Methods: This study included 51 female patients with BPD between the ages of 18 and 35 years, who were not using psychotropic medication, and 51 healthy controls matched for age and educational level. Optic nerve sheath diameter, intraocular pressure, and tear break-up time were measured, and trauma history was evaluated using the Childhood Trauma Questionnaire-Short Form. Independent t-test and Pearson correlation analysis were used in statistical analyses. Results: Patients with BPD were found to have significantly higher mean optic nerve sheath diameter scores (left: 3.94 +/- 0.43, right: 3.97 +/- 0.47) compared with healthy controls (left: 3.76 +/- 0.44, right: 3.78 +/- 0.45) (p < 0.05). The groups showed no significant difference in intraocular pressure and dry eye parameters (p > 0.05). A significant positive correlation was noted between emotional abuse scores and the optic nerve sheath diameter of the left eye in patients with BPD (p < 0.05; r = 0.364). Conclusions: An increased optic nerve sheath diameter may be a potential peripheral biomarker reflecting chronic stress or changes in intracranial physiology in patients with BPD. This increase is particularly associated with a history of emotional abuse. Ophthalmological parameters may contribute to understanding the neurobiological basis of BPD and serve as peripheral biomarkers or indicators of neurobiological changes.

    The Relationship among Dyadic Adjustment and Disease Burden in Patients with Bipolar Disorder and Their Spouses

    (1) Background: Spouses of individuals with bipolar disorder (BD) experience significant burdens, and the perception of the burden may affect dyadic adjustment. We aimed to investigate the sexual functions, alexithymic traits, marital satisfaction, and burden in patients with BD and their spouses. We also aimed to assess the mediating role of sexual functions and alexithymia in the relationship between burden and dyadic adjustment. (2) Methods: We included 81 patients with BD type 1 (40.69 +/- 8.55 years, 65.4% female, and 34.6% male) and their healthy spouses (40.95 +/- 7.30 years, 34.6% female, and 65.4% male) and 78 healthy controls (38.90 +/- 5.88, 48.7% female, and 51.3% male). The participants were evaluated using the Golombok-Rust Inventory of Sexual Satisfaction (GRISS), Dyadic Adjustment Scale (DAS), Hamilton Depression Rating Scale (HDRS), Toronto Alexithymia Scale-20 (TAS-20), and Burden Assessment Scale (BAS). (3) Results: The GRISS scores of the control group were significantly lower than the spouses and BD groups. The DAS total score of the control group was significantly higher than that of the spouses and BD groups. Regression analyses revealed that TAS, GRISS, and HDRS scores were associated with DAS scores in the BD group. In the spouse group, TAS and BAS scores were associated with DAS scores. The GRISS scores partially mediated the relationship between dyadic adjustment and burden in the spouses of patients with BD. (4) Conclusions: Mental health professionals should regularly scan caregivers' perceptions of burden. Appropriate psychosocial interventions could help spouses of patients with BD to cope better with the burden and improve dyadic adjustment.
