    Bacteriological profile of diabetic foot ulcers and analysis of serum meteorin levels

    ObjectiveWith the increase in the incidence of diabetes mellitus (DM) in recent years, diabetic foot ulcers (DFU), which are common and serious chronic complications of diabetes, have also become widespread. DFU is highly associated with a significant deterioration in quality of life as well as increased morbidity and mortality. Meteorin is a potent neurotrophic growth factor and shows antiangiogenic, antihyperalgesic, antinociceptive and neuroprotective effects. This study aimed to determine the possible relationship between meteorin, diabetes and diabetic foot ulcer by comparing the serum meteorin levels of healthy control group, DM patients and patients with diabetic foot ulcers.MethodsOur study included a total of 62 diabetic patients, 31 of whom had DFU, and 29 healthy individuals as a control group. Meteorin levels of the participants were measured using ELISA method in serum samples. Other laboratory and epidemiological data of the patients were obtained from the hospital database.ResultsIn the wound cultures taken from patients with DFU, the most commonly isolated bacteria were Staphylococcus aureus and Pseudomonas aeruginosa. Serum meteorin levels were found to be statistically significantly higher in diabetic patients as compared to the healthy control group, and among diabetic patients, those with DFU had significantly higher levels compared to those without DFU. A positive significant correlation was found between meteorin level and age, HbA1c, WBC, urea, sedimentation, CRP and ferritin.ConclusionThe results of our study, aimed at better understanding the biological functions and potential clinical applications of meteorin, suggest that meteorin could potentially be used as a biomarker for the development of DFU.

    mTOR signaling pathway genes effect in COVID-19 infection

    Coronavirus disease 2019 (Covid-19) is an infectious disease that causes severe acute respiratory illness caused by coronavirus 2 (SARS-CoV-2). SARS-CoV-2 uses host-specific metabolic pathways, including mTOR. The mTOR pathway is hyperactive in viral respiratory tract infections and contributes positively to viral replication. 100 samples were evaluated, 50 patients (Female=23, Male=27), and 50 controls (Female=29, Male=21). The patients were individuals who were COVID-19 positive. We detected expression changes of 5 genes in mTOR complex 1 (mTORC1) and mTOR complex 2 (mTORC2) (MLST8, mTOR, RPTOR, MAPKAP1 and RICTOR). Serum samples were obtained from all patients. The expression changes of mTORC1 and mTORC2 Complex genes were evaluated with Real-time PCR method. Receiver operating curve (ROC) analysis was performed to define the diagnostic power of these genes. Expression changes of five genes in the mTORC1 and mTORC2 complex were statistically significant (p =0.001) and upregulated in serum. The area under the ROC Curve values indicating the diagnostic power of genes were 0.948, 0.771, 0.851, 0.798, and 0.805, respectively. These genes may be candidate for treatment targets. The high discriminative power of these genes in patients from controls indicates their diagnostic potential in serum samples.

    Pasteurella multocida Pneumonia with Hemoptysis in an Immunocompetent Case

    Pasteurella species are gram-negative bacilli found in healthy pets' oropharynx and gastrointestinal tract flora. In humans, skin and soft tissue infections develop most frequently with the bite or scratching of animals such as cats or dogs. At the same time, they cause infections in the respiratory tract, mainly in patients with chronic lung disease or immunosuppressive patients. In this case report, a rare case of pneumonia caused by P.multocida bacteria in a patient with bronchiectasis was presented. A young male patient was admitted to the emergency department of our hospital with complaints of hemoptysis, cough with phlegm, and weight loss. The patient's blood pressure was 140/82 mmHg and SO2= 94%. Rales and rhonchi were detected in the lower left lung during the examination. Standard thorax tomography revealed prominent cystic structures and pneumonic infiltrates in the left lower lobe. Laboratory findings were normal. The Coronavirus disease-2019 (COVID-19) quantitative real-time polymerase chain reaction (qRt-PCR) test was found to be negative in the nasopharyngeal swab sample taken from the patient. Fiberoptic bronchoscopy was performed on the patient to investigate the presence of endobronchial lesion or foreign body aspiration. Culture and cytological evaluation was requested from the bronchial lavage taken. Gram-negative coccobacilli were seen among dense polymorphonuclear leukocytes in the Gram stain of the sample. Acid-fast bacilli were not detected with Ehrlich Ziehl Neelsen stain. In the lavage culture evaluated after 24 hours, colonies growing in blood and chocolate media were stained and gram negative coccobacilli were observed. The isolate was identified as 96.0% P.canis with the automated Vitek 2 (Biomerieux, France) system. It was determined that the isolate was susceptible to levofloxacin, trimethoprim-sulfamethoxazole, amoxicillin-clavulanic acid, penicillin, ciprofloxacin and cefotaxime in the antibiogram performed by disc diffusion test according to EUCAST v13.0 guideline criteria. Sequence analysis of the isolate obtained from the culture was performed on the ABI Prism 310 Genetic Analyzer (Applied Biosystems, USA). Sequence analysis of the isolate revealed 99.85% homology with P.multocida (GenBank accession no: NG_115137.1). Although Pasteurella multocida pneumonia is not commonly observed, the presence of underlying bronchiectasis in this patient facilitated the establishment of the bacteria. In order not to miss the diagnosis of pneumonia due to P.multocida, microbiological evaluation and molecular typing should be performed in the samples taken from the respiratory tract in patients with chronic respiratory diseases such as bronchiectasis.
