    Association between attention deficit hyperactivity disorder and chronotype in adults with epilepsy

    Purpose: This study aimed to determine the chronotype characteristics in adults with epilepsy, examine the relationship between chronotype and Attention Deficit Hyperactivity Disorder (ADHD), and compare ADHD symptom levels between patients with epilepsy and healthy controls. Materials and Methods: The study included 90 patients with epilepsy and 70 healthy controls, aged between 18 and 65 years. Sociodemographic and clinical data (seizure status, EEG, and MRI findings) were collected from the participants. The Morningness-Eveningness Questionnaire (MEQ), Adult ADHD Diagnosis and Evaluation Inventory, and Adult ADHD Self-Report Scale (ASRS) were administered. Results: A higher proportion of intermediate chronotypes was observed in the epilepsy group (68.9%) than in the control group (51.4%). ADHD symptom scores were higher in patients than in controls. Among patients with epilepsy, individuals with intermediate and evening chronotypes had significantly higher ADHD symptoms than those with morning chronotype. Although individuals with an evening chronotype had higher ADHD symptom levels than those with an intermediate chronotype, this difference was not statistically significant. Conclusion: The findings suggest that ADHD symptoms become more pronounced in patients with epilepsy as chronotype characteristics shift toward eveningness. Circadian features should be considered in the evaluation and management of ADHD comorbidity in adult patients with epilepsy.

    Çocuklar İçin Özel Gereksinim Raporu (ÇÖZGER) Almak Üzere Sağlık Kuruluna Başvuran Hastaların ÇÖZGER Yönetmeliğine Göre Hareket Gelişim Alanının İncelenmesi: Kesitsel Bir Araştırma

    Hastanemize çocuklar için özel gereksinim raporu (ÇÖZGER) almak\rüzere başvuran hastaların; demografik özellikleri, hareket gelişim alanı ile ilgili tanı dağılımları ve sağlıkla ilgili özel gereksinimlerinin belirlenmesi amaçlanmıştır. Gereç ve\rYöntemler: Sağlık kuruluna ÇÖZGER almak amacıyla başvuran, 472 hasta retrospektif olarak incelendi. Hastaların cinsiyet, yaş, özel gereksinim durumu, başvuru nedenleri ve hareket gelişim alanına ait veriler kaydedildi. Tüm lokomotor sistem için hareket\rgelişim alanı, romatolojik hastalıklar alanı, kalıtsal doğmalık hastalıklar alanı, metabolizma alanı ve sinir sistemi alanındaki etkilenme kaydedildi. Bulgular: Sağlık kuruluna\rbaşvuran hastaların %39’u (n=184) kız, %61’i (n=288) erkek cinsiyetteydi. Özel gereksinim alanı değerlendirmesinde hareket gelişim alanında %40,1 (n=189) oranında\rhasta vardı. Bu alanda en fazla serebral palsi tanılı hasta oranı %18,4 (n=87) idi. Bunu\rsırasıyla %5,2 (n=25) ile motor fonksiyonlarda özel gelişimsel bozukluk, %1,2 (n=6) ile\rbrakial pleksus hasarı, %1,4 (n=7) ile parapleji izlerken, konjenital diğer malformasyonlar/sendromlar; spina bifida, tethered kord sendromu, skolyoz, artrogripozis multipleks konjenita tanılarında %13,5 (n=64) oranında hastada vardı. Hastaların özel\rgereksinim düzeyi en çok özel koşul gereksinim var %39 (n=188) şeklindeydi ve\r%91,3’ünün (n=431) özel gereksinim ihtiyacı vardı. Özel gereksinim düzeyi kızlar ile\rerkekler arasında farklıydı (p<0,023). Sonuç: Çocukluk çağının işlevleri, etkinlikleri\rve sosyal yaşamdaki özel gereksinimlerinin doğru belirlenmesi çok önemlidir. Hareket\rgelişim alanı, psikiyatrik hastalıklardan sonra en çok etkilenen gelişim alanıdır (%40,1)\rve fonksiyonel kullanımının sağlanması, büyüme gelişme çağındaki çocuklarda kritik\röneme sahiptir, ayrıca erişkinlerden farklı bir disiplinle yaklaşım gerektirir. Konuyla\rilgili hekimlerin ÇÖZGER’in bu alanında bilgi sahibi olması hem rehabilitasyon hem\rde sosyal destek gereksinimi açısından önemlidir.

    Deep feature extraction based brain image classification model using preprocessed images: PDRNet

    Background: Stroke is a neurological condition that occurs when cerebral vessels become blocked and have reduced blood flow. This research proposes a hybrid deep feature-based feature engineering model to achieve high classification performance. Materials and method: In this research, three brain magnetic resonance image datasets were used to test the proposed model. A deep feature engineering model has been proposed to deploy the raw MRI and four pre-processing algorithms: GradCAM, histogram-matching, canny edge detection, and Locally Interpretable Model-Agnostic Explanations(LIME). The deep features have been extracted using Resnet101 and DenseNet201 pre-trained convolutional neural networks (CNN). Thus, this model is titled preprocessing based DenseNet and ResNet (PDRNet). The iterative neighborhood component analysis (INCA) function selects the most suitable features. These features are trained and validated using support vector machine (SVM) classifiers. Iterative Majority Voting (IMV) has been applied to the results obtained from the SVM. The best classification result has been selected by deploying IMV. Results: Our proposed PDRNet achieved a classification accuracy of 97.56% for Dataset 1, 99.32% for Dataset 2, and 99.16% for Dataset 3. The success of the presented model is demonstrated using these calculated accuracies. Conclusions: Our proposed hybrid deep feature model was tested on two datasets with two and four classes. It has also been compared to other state-of-art deep learning-based models, and our model performs better. These results and findings clearly demonstrate the success of the introduced hybrid deep feature engineering method.

    Reversible lesion of the corpus callosum associated with COVID-19: A case report

    Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) may affect the central nervous system and peripheral nervous system. Major central nervous system manifestations of SARS-CoV-2 infection include seizures, meningoencephalitis, ischemic stroke, anosmia, and hypogeusia. The reversible splenial lesion syndrome was first described in 2004. Although reversible splenial lesion syndrome was initially recognized as a benign phenomenon, a second type of reversible splenial lesion syndrome was identified in later years, which has a poorer prognosis and potentially serious sequela. Reversible splenial lesion syndrome can be caused by numerous etiologies including viruses. In this report, we present a rare case of COVID-19 with reversible splenial lesion, who presented with ataxia and dizziness.

    Sağlık Kuruluna Başvuran Nörolojik Tanılı Hastaların Demografik ve Klinik Verilerinin Retrospektif Değerlendirilmesi

    Amaç: Engellilik oluşturan nörolojik hastalıklar, pahalı tedavi\rmaliyetleri ve belirgin iş gücü kaybına neden olabilirler. Bu çalışmada,\rOcak-Aralık 2017 tarihleri arasında hastanemizin Engelli Sağlık Kuruluna\rbaşvuran nörolojik tanılı hastaların demografik ve klinik verilerini\rdeğerlendirmeyi amaçladık. Gereç ve Yöntem: Ocak-Aralık 2017\rtarihleri arasında hastanemizin Engelli Sağlık Kuruluna engelli raporu\riçin başvuran hastalar retrospektif olarak taranmış; yaş, cinsiyet, tanısı,\rbaşvuru nedeni ve özür oranı tespiti yapılmıştır. Bulgular: Çalışmaya\r1.870 nörolojik tanısı olan hasta dâhil edilmiştir. Kadın yaş ortalaması,\rerkeklerden anlamlı derecede yüksek tespit edilmiştir (p<0,001). En sık\rbaşvurunun, bakım hizmeti için olduğu gözlenmiştir. En sık başvuru\rtanısı demans tanısı ile olup, yaş ortalaması ve engellilik oranı en fazla\rdemansta tespit edilmiştir. Hem Parkinson hastalığında hem de demansta\rerkeklerin özürlülük oranı, kadınlarınkinden daha fazla bulunmuştur;\rfakat bu fark sadece demans tanısında istatistiksel açıdan\ranlamlı bulunmuştur. En genç yaş grubu ve en az engellilik oranları, serebral\rpalsi ve epilepsi grubunda izlenmiştir. Demans hastalarının\r1/3’ünde işitme kaybı olduğu tespit edilmiştir. Serebrovasküler hastalıklara\ren sık eşlik eden hastalık %30,5 sıklıkla hipertansiyon iken; en\raz eşlik eden hastalık %0,4 sıklıkla atriyal fibrilasyon olarak tespit\redildi. Sonuç: Bu ve benzeri çalışmalar, engelliğe neden olan hastalıkları\rbelirleyerek, ilgili hastalıkların profilaksi ile önlenmesine; hastaların\rrehabilitasyon çalışmaları ile topluma kazandırılmasına yol\rgösterici olacak ve toplumsal maliyeti azaltacaktır.

    The use of non-steroidal anti-inflammatory drugs in cerebrovascular diseases

    Cerebrovascular diseases (CVD) are a group of diseases resulting from ischemic or hemorrhagic conditions, causing neurological deficits. In developed countries, CVD are the third leading cause of death and the leading cause of disability. In ischemic and hemorrhagic conditions, nonsteroidal and anti-inflammatory drugs (NSAIDs) are commonly used as antiplatelet, anti-inflammatory, and analgesic drugs. On the other hand, these drugs have been associated with increased risk of hemorrhage and thrombosis. One of cyclooxygenase-1 (COX-1) inhibitor, acetylsalicylic acid (ASA), is the most widely used agent in the acute treatment and prophylaxis of ischemic CVD. Moreover, ASA catalyzes the conversion of arachidonic acid into prostaglandin H2 through irreversible acetylation of COX-1 and cyclooxygenase-2 (COX-2), thereby reducing platelet aggregation. Another subgroup of NSAID is known as COX-2 selective NSAIDs including celecoxib, rofecoxib, and valdecoxib. These agents, however, have been removed from the market after being reported to increase the incidence of thrombotic stroke and myocardial infarction. A previous meta-analysis indicated that diclofenac, similar to coxibs, increases the risk of ischemic CVD compared to the nonuse of NSAIDs and also suggested that naproxen appears to have a better CVD safety profile compared to other NSAIDs. As NSAIDs are a chemically heterogeneous group of compounds, the use of each compound may have a different effect on not only thrombotic but also hemorrhagic risk factors. A recent study conducted systematic review to evaluate the risk of CVD associated with the use of individual NSAIDs and revealed that the use of diclofenac and meloxicam is associated with an increased risk of hemorrhagic CVD. The authors also noted that the use of these two NSAIDs leads to vasoconstriction by inhibiting the COX-2 in the kidney and also increases salt and water reabsorption, thereby increasing the risk of hypertension and CVD. In light of these notions, it is safe to assert that when prescribing NSAIDs, these drugs may have procoagulant effects, cause hypertension by increasing fluid and salt retention, may interact with drugs used in CVD prophylaxis. So it should be recognized that these drugs are likely to cause an increased risk of thrombosis and hemorrhage. © 2021 by Nova Science Publishers, Inc. All rights reserved.
