Cardiac Findings on Non-Contrast Thoracic Tomography in Patients with Acromegaly
(2022) keskin, lezan; Ilgar, Mehtap; YAPRAK, BÜLENT; Akçiçek, Mehmet; Aslantürk, Okan
Aim: Acromegaly occurs as a result of excessive and permanent secretion of growth hormone from the pituitary. Mortality is mostly related to cardiovascular system involvement. In our study, we aimed to evaluate the correlation between epicardial fat volume (EFV) and growth hormone level in thorax computed tomography in patients with acromegaly and coronary artery calcification, pulmonary artery diameter, ascending aorta diameter, cardiothoracic ratio (CTO) measurements with the control group patients. Method: Our study was retrospective and included 16 patients with acromegaly who were previously diagnosed and treated by the endocrinology clinic and a control group consisting of 32 patients matched for gender and age.In thorax CT, EFV measurement of the patients was performed and main pulmonary artery diameters, ascending aorta diameters, cardio thoracic ratios, presence of coronary artery calcification were evaluated. Results: The number of patients with large ascending aorta was higher in patients with acromegaly and it was statistically significant (p=0.041). Although the rate of patients with large main pulmonary artery diameter was higher in patients with acromegaly, no significant difference was found between the groups (p=0.355). There was no significant difference between the groups in terms of increased CTO (p=0.818) and coronary artery calcification (p=0.157). Conclusion: In our study, a difference was found between the acromegaly and control group patients only in terms of ascending aorta diameters, but no significant difference was found in terms of other parameters. We think that regular follow-up and treatment of patients is effective in this result. Cardiovascular risks can be reduced in patients with acromegaly with early diagnosis, regular follow- up and treatment.

(2022) YAPRAK, BÜLENT; Sahin, ibrahim; evren, bahri; keskin, lezan; erbay, lale gonenir
Aim: Obesity is a risk factor for sexual dysfunction. The aim of this study was to determine the frequency of sexual dysfunction in patients diagnosed with morbid obesity. Methods: The patient group included in the study consisted of 78 morbidly obese patients with BMI?40 kg/m2 and 68 healthy individuals with normal BMI. The data were obtained by using the sociodemographic information form filled by the participants, Beck anxiety scale, Beck depression scale and Golombok Rust Sexual Satisfaction Inventory. Results: Obese individuals were found to have sexual dysfunction compared to individuals with normal body BMI (p<0.05). When the Golombok Rust subscale scores of obese men were compared to those of normal-weight individuals, a significant deterioration was found in all subscale scores, including frequency, communication, satisfaction, avoidance, touch, impotence and premature ejaculation. In addition, the anxiety and depression scores of obese individuals were shown to be higher than those of normal-weight individuals (p<0.05).Conclusion: Morbidly obese individuals should be evaluated in terms of sexual functions. We believe that regulating obesity treatment, including possible treatment for sexual dysfunction, will increase the success rate and enhance the quality of life for patients.

Examination of eating disorder risk and effective factors in university students
    (2023) Aytaş, Özgür; YAPRAK, BÜLENT
The aim of this study; determine the eating attitudes and behaviors of university students, who are a sensitive population in terms of eating disorders, and to examine some related factors. A structured questionnaire and face-to-face interviews collected the data. The data collection form has two sections: one for sociodemographics and health behaviors, and one for dietary habits and eating disorders using eating disorders scale (REZZY). The questionnaire explained voluntary study participation. The mean age of the 762 university students included in the study was 23.44±5.33 years and 58.40% were female. Variables that one- off analyses suggested might be connected to the risk of developing eating disorders were examined using the logistic regression model. There was a significant and positive correlation between the REZZY-Total (REZZY-T) score, bulimia and food preoccupation (EAT-1) and dieting (EAT-2) (p<0.01). The study group's female had an eating disorder risk that was 2.1 times greater than that of the male. As students advance through their educational phase, the risk of eating disorders significantly declines (p<0.001). Positive factors reduced the risk of eating disorders by 1.4 times, whereas negative factors roughly doubled it (p<0.001). Impaired eaters eat when they're not hungry, restrict their diet, obsess over their diet, and use food to cope with negative emotions. This will cause emotional, physical, and social problems and lower their quality of life. To prevent disease, improve mental health, and reduce social issues, examine eating behavior.

Pain behaviors and hemodynamic parameters of intubated and sedatized intensive care patients during aspiration
(2021) Menekli, Tugba; Doğan, Runida; YAPRAK, BÜLENT
The study was conducted in a descriptive type to evaluate the pain behavior and hemodynamic parameters during aspiration of intubated and mildly sedatized patients in the intensive care unit. The study sample consisted of 100 intubated and sedatized patients who were hospitalized in the Anesthesia Intensive Care Unit of Harran University Faculty of Medicine Hospital between February 2018 and June 2018. \"Patient Information Form\", \"Hemodynamic Parameter Form\", \"Behavioral Pain Scale (BPS)\", \"Ramsay Sedation Scale\" and \"Glasgow Coma Scale (GCS)\" were used to collect the data. There is a significant difference between the BPS total and subscale scores, mean \"systolic and diastolic blood pressure\", \"heart rate\", \"respiratory rate\" and \"SpO2\" before, during and after aspiration (p=0.0001), it was determined that the difference was due to the average score after aspiration (p=0.0001). While the SpO2 averages of intubated and sedatized intensive care patients decrease during aspiration, the averages of the BPS scores and other hemodynamic parameters increase.

The correlation between vitamin D levels and inflammation, as well as the Prognostic Nutritional Index (PNI), in older adults
    (2023) Arslan, Nurgül; YAPRAK, BÜLENT
Aim: Older adults’ populations often have insufficient vitamin D (vit-D), which may contribute to autoimmune diseases and inflammation. Few studies have examined vit-D levels and immune function in humans. The study looked at vit-D levels and inflammatory markers in older adults. Materials and Methods: Retrospective research was done. The hospital’s records were searched for the study’s data. Participants were 65-year-olds who applied to the geriatrics and internal medicine outpatient clinic between May 2020 and May 2022. Each person’s vit-D nmol/L, CRP(mg/L), lymphocyte/monocyte, neutrophil/lymphocyte, platelet/lymphocyte, ferritin, and prognostic nutritional index (PNI) levels were measured. Results: A total of 427 outpatients who applied to the geriatrics and internal medicine outpatient clinic between May 2020 and January 2022 were analyzed. Among these patients, 103 did not meet study criteria. A total of 324 participants who met the study criteria were evaluated and the participants’ mean age was 73.21±5.24 years. The mean level of vit-D found in older adults was 14.18±6.14 nmol/L. It was discovered that there was a negative correlation that was statistically significant between the individuals’ levels of vit-D and the ratio of CRP(mg/L) to MPV(fL) (p<0.05). Vit-D and PNI were found to have a correlation of r=0.205, which was found to be positive and significant (p<0.05). Logistic regression analysis was performed with vit-D level and related factors. Every 0.721 increase in PNI result in an additional unit of 25(OH) vit-D. It was found that the CRP (mg/L) variable contributed significantly to the model, and it was discovered that the presence of 1 unit of 25(OH) vit-D was increased for every 0.202 unit decrease (p<0.05). Conclusion: Insufficient vitamin 25(OH)D is found in the older adults population. 25(OH) vit-D has a negative relationship with CRP and MPV, but a positive relationship with PNI, an indicator of nutritional status. The study suggests that vit-D supplements should be provided if an older adult has adequate vit-D.

The relationship between death risk and nutritional factors in individuals with acute coronary syndrome
(2022) YAPRAK, BÜLENT; Arslan, Nurgül; AKBULUT, GAMZE
There is a relationship between low food intake and risk of death in patients with acute coronary syndrome. In this study aimed to examine the relationship between The Global Registry of Acute Coronary Events (GRACE risk score) and food intake.: 140 men over 35 years of age with Acute Coronary Syndrome were included in the study. The 1-day energy and nutrient consumption of individuals was determined using a 24-hour food consumption recording method. Based on the GRACE risk score classification system, the patients were divided into four quartiles (89.5?Q1?72, 102.5?Q2?89.5, 117.75?Q3?102.51, 152?Q4?117.76). The macro and micronutrient intakes of individuals were compared between the four groups. Nutrient intake decreased in the group with the highest grace risk score. inadequate nutrient intake further increased the GRACE risk score. A negative correlation was found between the GRACE risk score and macro and micronutrients (p<0.05). We found that the increase in the GRACE risk score decreased the food intake otherwise a low level of food intake increased the risk of death.

(2023) ALATAŞ, HACER; Saka, Mendane; Arslan, Nurgül; YAPRAK, BÜLENT; Otlu, Önder; Pembegul, Irem
Purpose: Oxidative stress increases in uremic conditions such as kidney failure and during hemodialysis. In this study, it was aimed to examine the relationship between the total antioxidant capacity of the diet and the serum oxidative stress (sOSI) index in hemodialysis patients. Materials and Methods: In this study conducted with 82 individuals (41 patients and 41 controls). Demographic characteristics and anthropometric measurements of the individuals were taken by researchers. In order to determine the nutritional status of the individuals, 3-day food consumption records were taken and the total antioxidant capacity of the diet was calculated. Results: The mean age of the individuals participating in the study was found to be 54.01±2.24 years. Hemodialysis treatment mean for 4.8±3.12 years. While 41.46% of hemodialysis patients were moderately malnourished. Dietary Oxygen Radical Absorption Capacity (dORAC) (?mol) measurement result was found to be 2415.3±1073.45, 5468.4±2393.85 in the hemodialysis and control groups, respectively. In the multiple logistic regression analysis performed, the decrease in body mass index (OR 2.21 95% CI 1.050-2.317) and triceps skinfold thickness values (OR 3.22 95% CI 1.722-4.001) in hemodialysis patients led to an increase in the sOSI value. It has been observed that when dietary protein (OR 1.23 95% CI 1.003-3.189), vitamin C (OR 2.88 95% CI 1.004-3.106) and dTAC (OR 2.04 95% CI 1.967-2.001) increase, the sOSI value decreases in hemodialysis patients. Conclusion: There is a positive relationship between diet total antioxidant capacity and serum antioxidant level, and it was determined that the dietary antioxidant capacity of hemodialysis patients was significantly lower than that of healthy individuals. According to the results of the study, it is recommended to monitor the food consumption status of hemodialysis patients in order to increase the dietary antioxidant capacity.

Viral respiratory tract infection in Gaucher disease, a rare disease: A case study of 24 patients in Malatya, Türkiye
(2023) YAPRAK, BÜLENT; ALADAĞ, HÜLYA; Aladag, Murat
Aim: People with chronic diseases have a higher rate of morbidity and mortality from SARS-CoV-2 infection. Unknown is how SARS-CoV-2 affects rare disease populations like Gaucher disease. The purpose of this research was to investigate the effects of SARS- CoV-2 infection has on Gaucher patients. The only study conducted in Turkey so far is this one. Materials and Methods: Patients with Gaucher disease were included in the study between August 2020 and September 2022. Patients diagnosed with Gaucher disease were questioned one-on-one to determine if they had come into contact with another person who had COVID-19 symptoms or another patient who had COVID-19. The patients were questioned and their answers were recorded regarding their GBA genotype, previous splenectomy history, whether or not they received treatment, and whether or not they had a SARS-CoV-2 test. Results: Eight of our patients reported having contact with someone who had COVID-19 infection or was suspected of having the infection, and in three of these cases (37.5%) the patient reported having at least one symptom of infection. Out of the 18 patients who were tested, 4 (22.2%) had a positive result. Positive test results were associated with more symptoms in patients (3.8 vs 0.3, p0.001) than negative test results. In Gaucher patients, symptoms or positive test results were not correlated with age, gender, BMI, comorbidity, genotype, previous splenectomy, or enzyme replacement therapy. All of our patients only received supportive care; none of them required intensive care or specialized treatment. Conclusion: Our research revealed that there was a low risk of serious side effects in other chronic patients following SARS-CoV-2 infection in Gaucher patients. In this regard, studies with larger case series are required
