Associations among chondromalacia patella and meniscopathy in youthful, active individuals with chronic anterior knee pain
    (2022) Akçiçek, Mehmet; Arı, Bünyamin
Chronic forms of anterior knee issues are an increasingly more common condition from the clinical standpoint, mostly within active individuals. Incidence and risk factors\rhave not been clearly defined of yet, though, patellar chondromalacia is one of the most likely causes. In our study, we aimed to investigate the possible relationship\rbetween patellar chondromalacia and meniscopathy in young and active patients without tibiofemoral joint arthrosis and patellar instability. Patients between the ages\rof 18-39, complaining of anterior knee discomfort and who had undertaken MRI between April 1 and May 31, 2021 at Malatya Training Research Hospital, Radiology\rDepartment were screened. Conditions such as significant patellar tilt/shift/translation, alta/baja and trochlear dysplasia were studied and measurements taken according\rto the methods specified in the literature. In this respect, patients outside of normal limits, those with osteochondoral lesions in the tibiofemoral joint or patients admitted\rafter acute trauma were disqualified from the study. 337 receipients of MRIs of the knee, with chondromalacia and/or meniscopathy were reviewed. The average or mean\rage of the group was calculated as 28.66 ± 6.40, with 52.2% (n=176) between the ages of 18-29, and 61.1% (n=206) were women. Chondromalacia was present in 40.7%\r(n=137), lateral meniscal degeneration in 58.5% (n=197), medial meniscus degeneration in 82.8% (n=279), a lateral meniscus tear in 4.2% (n=14), a medial meniscal\rtear in 12.5% (n=42). In this study, no difference was found in terms of the coexistence and/or frequency of medial or lateral meniscopathy and patellar chondromalacia.

Cardiac Findings on Non-Contrast Thoracic Tomography in Patients with Acromegaly
(2022) keskin, lezan; Ilgar, Mehtap; YAPRAK, BÜLENT; Akçiçek, Mehmet; Aslantürk, Okan
Aim: Acromegaly occurs as a result of excessive and permanent secretion of growth hormone from the pituitary. Mortality is mostly related to cardiovascular system involvement. In our study, we aimed to evaluate the correlation between epicardial fat volume (EFV) and growth hormone level in thorax computed tomography in patients with acromegaly and coronary artery calcification, pulmonary artery diameter, ascending aorta diameter, cardiothoracic ratio (CTO) measurements with the control group patients. Method: Our study was retrospective and included 16 patients with acromegaly who were previously diagnosed and treated by the endocrinology clinic and a control group consisting of 32 patients matched for gender and age.In thorax CT, EFV measurement of the patients was performed and main pulmonary artery diameters, ascending aorta diameters, cardio thoracic ratios, presence of coronary artery calcification were evaluated. Results: The number of patients with large ascending aorta was higher in patients with acromegaly and it was statistically significant (p=0.041). Although the rate of patients with large main pulmonary artery diameter was higher in patients with acromegaly, no significant difference was found between the groups (p=0.355). There was no significant difference between the groups in terms of increased CTO (p=0.818) and coronary artery calcification (p=0.157). Conclusion: In our study, a difference was found between the acromegaly and control group patients only in terms of ascending aorta diameters, but no significant difference was found in terms of other parameters. We think that regular follow-up and treatment of patients is effective in this result. Cardiovascular risks can be reduced in patients with acromegaly with early diagnosis, regular follow- up and treatment.

    Cranial Trauma
(Nova Science Publishers, Inc., 2025) Akçiçek, Mehmet
Cranial trauma can cause traumatic brain injury (TBI), which is one of the leading causes of mortality and morbidity in the world. TBI can develop with both primary and secondary injuries. Primary injuries can include contusions, hematomas, and diffuse axonal injuries, which can be either intra-axial or extra-axial lesions such as epidural-subdural hematomas, and subarachnoid hemorrhages. Cerebral edema, ischemia, herniation, hydrocephalus, and encephalomalacia are secondary injuries. In TBI, clinically determined according to the Glasgow Coma Scale, imaging techniques have an important role in identifying existing injury findings and complications. Today, computed tomography is the main method used for imaging cranial trauma. It is known that magnetic resonance imaging is highly sensitive to pathological changes associated with mild TBI and is useful in assessing the severity and prognosis of the injury. In recent years, studies have also been conducted on the use of functional imaging techniques such as diffusion tensor imaging, perfusion imaging, and spectroscopy, as well as molecular imaging methods in TBI. It is important for radiologists and clinicians, who have a critical role in the diagnosis of brain injury and related complications resulting from cranial trauma, to make the correct decisions about the preferred modalities for imaging both primary and secondary injuries. © 2025 Nova Science Publishers, Inc. Evaluation of Cervical Spinal Cord Atrophy by Magnetic Resonance Imaging in Patients with Multiple Sclerosis
(2022) Akçiçek, Mehmet; Ilgar, Mehtap; ünlü, serkan
Background: Multiple sclerosis (MS) is an inflammatory disease of the brain and spinal cord (SC) that leads to demyelination and neurodegeneration. The relationship between cervical cord atrophy and disability in MS patients was demonstrated. Methods: The examinations were made with MR-Philips Medical System 1.5 tesla device in sagittal and axial planes with T1-WI and T2-WI. Images were evaluated by two experienced radiologists through the hospital image archive system. Results: In the sagittal plane measurement from the C3 vertebra level, the mean SC measurement of the patients in the MS group was 7.3 ± 0.7 millimeters (mm), while the mean of the control group was 8.3 ± 0.6. In the sagittal plane measurement from the C6 vertebra level, the mean SC measurement of the patients in the MS group was 6.9 ± 0.7 mm, while the mean of the control group was 7.8 ± 0.5. In the measurements made at all levels, SC thicknesses were lower in MS group patients compared to control group patients, and this decrease was statistically significant. Conclusion: In daily practice, cervical spinal cord measurements in MS patients can be easily and quickly performed with conventional MRI in two dimensions.

(2024) Arpacı, Muhammed; TAŞ, FERHAT; Uzun, Gökçe Bağcı; Akçiçek, Mehmet; İnceoğlu, Feyza; DENIZ, Mine ARGALI; DEMİREL, EMRE
Bone fractures that occur in earthquakes threaten human life. The study aims to localize the fractures in earthquake victims according to anatomical regions in the earthquake centered in Kahramanmaraş 2023. The study included 137 patients aged between 1-90 years diagnosed with fractures by an orthopedic physician. Radiographs were taken of patients admitted to the emergency department after the earthquake. The fractures diagnosed patients were evaluated and classified in anatomical regions and fracture type. The fractures were observed in females with high rates of fragmented (49.6%) and s ingle fractures (75.9%). The fractures also occurred on the right side (58.4%), in the upper extremities (37.2%) and long bones (38.7%). In the specific anatomic region evaluation of fractures; high rates were observed in manus (19%), antebrachi (15.3%), crus (13.9%) and pes fractures (12.4%). In the region of the most common fragmentary fracture type (48.9%); omos (3%), brachium (1%), thorax (15%), viscerocranium (7%) and neurocranium (3%), in oblique fractures (25.5%); pes (26%), in transverse fractures (20.4%); antebrachi (21%), manus (32%) and vertebral column (4%), in spiral fractures; pelvis (14%), femur (14%) and crus (57%) region. We determined that fractures frequently occur on the right-side extremities and, besides sequentially high rates at manus, antebrachii, crus, and pes. We recommend providing information about the protection of these anatomical parts in the training given to people during the earthquake closure and post-earthquake panic times to reduce earthquake-related fractures.

IMAGING METHODS IN MUSCULOSKELETAL SYSTEM PAIN
(Nova Science Publishers, Inc., 2022) Akçiçek, Mehmet
Musculoskeletal system pain frequency increases with the aging population. Taking a detailed history and physical examination are the most important things for a correct diagnosis. Imaging should be considered as a tool to confirm the diagnosis. A number of imaging modalities are used to evaluate musculoskeletal system diseases. Despite the emergence of new imaging techniques for the assessment of musculoskeletal system pain, conventional radiology remains the cornerstone of diagnostic imaging. Radiography is the most advantageous and cheapest method in evaluating bone abnormalities such as fractures, dislocations, and osteoarthritis. Computed tomography can provide high-resolution, two- and three-dimensional images. Nuclear medicine applications are used in investigating stress fractures, occult fractures, bone tumors, or infections. Magnetic resonance imaging is used to evaluate soft tissue disorders, including ligaments, tendons, cartilage tissue, and masses. Ultrasonography is effective in evaluating effusions, cysts, ligaments, muscles, and tendons. Vascularization can also be identified with the use of Doppler. There is also the possibility to perform aspirations and injections under ultrasonography guidance. It is important to choose the most appropriate imaging method to accelerate the diagnosis and treatment. The imaging method to be preferred also changes depending on the structure of each joint and region investigated and the characteristics of the diseases considered in the differential diagnosis. © 2022 by Nova Science Publishers, Inc.

Investigation of the Effects of Hydronephrosis on Renal Morphometry and Volume Values
(Erciyes Üniversitesi, 2025) Akçiçek, Mehmet; Uzun, Gökçe Bağcı; Arpacı, Muhammed Furkan; Demirel, Emre; Pekmez, Hıdır
Hydronephrosis is a significant clinical condition characterized by dilatation of the renal collecting system, parenchymal thinning, and loss of volume due to increased intrapelvic pressure, leading to disruption of both the morphological and functional integrity of the kidney. In this study, renal volume and morphometric measurements were compared between individuals with and without hydronephrosis using computed tomography (CT) images, and the impact of hydronephrosis on renal volume and geometry was evaluated using three different volume measurement methods. This retrospective study included non-contrast abdominal CT scans obtained between 2017 and 2022. A total of 91 cases (50 with hydronephrosis and 41 without hydronephrosis) were evaluated. Renal volumes were calculated using semi-automatic segmentation with ITK-SNAP (Volsinak), manual area measurement with RadiAnt DICOM Viewer, and the ellipsoid formula method. Additionally, morphometric parameters such as renal thickness, width, length, polar distances (PSAD, PIAD), distances to the spinal medulla (PSMSD, PIMSD), and the angular measurement between the kidney and the transverse plane (BUTDANGLE) were assessed. The findings demonstrated significant differences in thickness, width, and BUTDANGLE parameters in hydronephrotic kidneys (p < 0.05), indicating that hydronephrosis disrupts not only kidney volume but also its geometric structure. A statistically significant difference was observed among the three volume measurement methods (p = 0.001), with the ellipsoid method tending to underestimate renal volume. In conclusion, this study reveals that hydronephrosis leads to marked structural and geometrical alterations in renal morphometry and volume values. It also highlights that three-dimensional semi-automatic segmentation-based methods provide more reliable assessments in renal volume analysis and contribute uniquely to the literature by incorporating novel morphometric parameters.

Is the effectiveness of ultrasound-guided percutaneous release therapy in trigger finger treatment affected by the stage of the disease?
(2022) Arı, Bünyamin; Altunkılıç, Tarık; Akçiçek, Mehmet; Deniz, Serdar
Abstract Aim: Trigger finger is a type of tenosynovitis that causes pain and dysfunction in the hand and fingers, especially in middle-aged people. We aimed to evaluate the effectiveness of ultrasound-guided (USG) percutaneous release for trigger finger according to the disease stage. Materials and Methods: USG percutaneous release was performed on 52 fingers of 52 patients who met the inclusion criteria. The patients with Quinnell stages 1 and 2 were grouped as group 1, and those with stages 3 and 4 were grouped as group 2. After treatment, our patients were followed up by measuring their pain with the visual analog scale (VAS) on the 7th day, during the 1st month, at the 6th month control visit and recording the time of the patient’s return to work. Results: In the analysis of the results from group 1 and group 2 together, there was a meaningful relationship between grade and VAS score. The mean of all four VAS scores in the second group was statistically significantly higher than in the first group (p < 0.001). In the comparison of the time to return to work according to grade, group 1 returned to work earlier than group 2 (1.67 ± 0.77 and 2.09 ± 0.83, respectively), but this difference was not statistically significant. Conclusion: The percutaneous release procedure, which is performed carefully after the location of the lesion has been precisely determined and marked by USG, is the preferred method, especially in the Quinnell stages 1and 2 of the disease.

The relationship between osteochondral lesion of the talus and the foot arch angles in adults: A retrospective study
(2023) boz, mehmet; Şahin, Abdullah Alper; Akçiçek, Mehmet
Aim: The current study aimed to compare the arch angles of the foot, Achilles tendinopa- thy, subcutaneous fat tissue thickness of the heel, and epin calcanei (EC) in patients with osteochondral lesion of the talus (OLT). Materials and Methods: The magnetic resonance images (MRI) of the ankle performed by the radiology clinic between March 2020 and October 2021 were examined. Based on the MRI findings, the patients diagnosed with OLT were included in the study group (n = 55) and those not diagnosed with OLT formed the control group (n = 118). All the patients included in the study were investigated for their demographic characteristics and presence of EC. Weight-bearing lateral X-ray images were used to measure the lateral talus- first metatarsal angle (Meary’s angle), lateral talocalcaneal angle (LTCA), and calcaneal inclination angle (also known as the calcaneal pitch). Results: The study and control groups had no significant difference in terms of gender distribution (P = 0.575). The patients in the study group had a significantly higher mean age than the control group (P < 0.001). Evaluation of the arch angles of the foot showed the patients with OLT to have higher LTCA and calcaneal pitch angles and greater sub- cutaneous fat thicknesses (p < 0.001, p < 0.001, and p = 0.004, respectively). In addition, the study group showed higher rates of patients with EC and Achilles tendinopathy (p = 0.076 and p = 0.019, respectively). According to the binary logistic regression analysis model, each unit of increase in the patients’ age increases the presence of osteochondritis dissecans (OCD) of the talus by 1.063 times (95% confidence interval (CI): 1.012-1.116; P = 0.015). Each unit of increase in LTCA increases the presence of OCD of the talus by 1.360 times (95% CI: 1.219-1.516; P < 0.001). Conclusion: Our study demonstrated that sex had no effect on OLT incidence, whereas changes in the LTCA and pitch angles elevated the risk of OLT. Furthermore, increased age was found to be associated with a higher risk of OLT formation. We also showed that Achilles tendinopathy and EC were significantly higher in patients with OLT. A considerable number of future studies are needed to establish whether this significant relationship is causal.

THE RELATIONSHIP BETWEEN THE SIZE AND LOCALIZATION OF THE URETERAL STONE AND THE DEGREE OF HYDRONEPHROSIS
(2023) Akçiçek, Mehmet; buğday, m. serdar
The aim of our research is to evaluate whether there is a relationship between the degree of hydronephrosis and, the size and location of the stones in the ureter in patients with ureteral stones, with computed tomography (CT). 105 patients who applied to Malatya Training and Research Hospital with the complaint of renal colic and underwent CT scan were included in the study. Hydronephrosis was evaluated by using the system developed by the Society of Fetal Urology. Stone sizes were grouped as <5 mm, 5 – 10 mm, and ? 10 mm. The location of the stones were indicated as proximal - middle - distal according to the anatomical parts of the ureters. 61 (58.1%) patients had stones in the distal ureter, 20 (19%) had stones in the middle ureter, and 24 (22.9%) had stones in the proximal ureter. It was determined that the stone size showed significant difference according to the degree of hydronephrosis (p<0.05). There was no significant relationship between the stone location being in the proximal, middle or distal parts of the ureter and the degree of hydronephrosis (p=0.241). While, as the size of ureteral stones increases the degree of hydronephrosis increases, there is no relation with the location of the stone and hydronephrosis.
