Öğe Anterior palatoplasty for selected mild and moderate obstructive sleep apnea: preliminary results(Springer, 2014) Ugur, Kadriye Serife; Ark, Nebil; Kurtaran, Hanifi; Kizilbulut, Gultekin; Yuksel, Alper; Gunduz, MehmetThe aim of this prospective study was to evaluate the long-term efficacy anterior palatoplasty (AP) technique in treatment of patients with mild to moderate obstructive sleep apnea (OSA). Forty-two patients were diagnosed with mild to moderate OSA. Participants were treated with AP for mild or moderate OSA. Patients were evaluated with one night polysomnography before the surgery and 24 months after the surgery. Patients completed Epworth sleepiness scale (ESS), snoring VAS (visual analog scale) before and 24 months after the surgery. Forty two patients with a mean age of 39.2 +/- A 7.6 were included study. Success rate was 57.1 %. Total apnea-hypopnea index (AHI) values significantly decreased after 2 years (p < 0.025). Non-REM AHI and supine AHI values significantly decreased after 2 years (p < 0.025). The oxygen desaturation index changes significantly decreased after AP (p < 0.025). Snoring VAS values significantly decreased after AP (p < 0.025). ESS scores of patients significantly decreased (p < 0.001). We believe that AP is an effective, inexpensive technique for mild and moderate OSA patients.Öğe Biomarkers in Hodgkin's Lymphoma(Crc Press-Taylor & Francis Group, 2014) Demir, Esin; Yilmaz, Burak; Gunduz, Mehmet; Gunduz, EsraLymphoma is a type of hematological cancer which occurs in the immune system, and specifically starts in lymphocytes. Lymphoma is basically of two types: Hodgkin's and non-Hodgkin's lymphoma (NHL). This chapter will specifically focus on Hodgkin's lymphoma (HL), whose subtypes are nodular lymphocyte-predominant Hodgkin's lymphoma (NLPHL) and classical Hodgkin's lymphoma (cHL). The characteristic cell types involved in these subtypes are different from each other. Biomarkers are another critical factor in making the correct identification of each subtype. Biomarkers can be categorized into different groups such as origin-related, chromosomal, cytokine, chemokine, and some important pathways-related markers. These biomarkers are not only important to distinguish subtypes but also critical to determine further treatment strategies for disease. We describe the chronological progression of this very rare syndrome from early childhood until adulthood in a patient who developed severe atrophy and lost one eye. We also discuss the aetiology and pathophysiology of this syndrome.Öğe Coffin-Siris syndrome with cafe-au-lait spots, obesity and hyperinsulinism caused by a mutation in the ARID1B gene(Int Research & Cooperation Assoc Bio & Socio-Sciences Advancement, 2016) Sonmez, Fatma Mujgan; Uctepe, Eyyup; Gunduz, Mehmet; Gormez, Zeliha; Erpolat, Seval; Oznur, Murat; Sagiroglu, Mahmut SamilCoffin-Siris syndrome (CSS) (MIM 135900) is characterized by developmental delay, severe speech impairment, distinctive facial features, hypertrichosis, aplasia or hypoplasia of the distal phalanx or nail of the fifth digit and agenesis of the corpus callosum. Recently, it was shown that mutations in the ARID1B gene are the main cause of CSS, accounting for 76% of identified mutations. Here, we report a 15 year-old female patient who was admitted to our clinic with seizures, speech problems, dysmorphic features, bilaterally big, large thumb, cafe-au-lait (CAL) spots, obesity and hyperinsulinism. First, the patient was thought to have an association of neurofibromatosis and Rubinstein Taybi syndrome. Because of the large size of the NF1 gene for neurofibromatosis and CREBBP gene for Rubinstein Taybi syndrome, whole exome sequence analysis (WES) was conducted and a novel ARID1B mutation was identified. The proband WES test identified a novel heterozygous frameshift mutation c.3394_3395insTA in exon 13 of ARID1B (NM_017519.2) predicting a premature stop codon p.(Tyr1132Leufs*67). Sanger sequencing confirmed the heterozygous c. 3394_3395insTA mutation in the proband and that it was not present in her parents indicating de novo mutation. Further investigation and new cases will help to understand this phenomenon better.Öğe Correlation of o-VEMP and v-HIT Test Results with Caloric Test Results iSn Peripheral Vestibular Disorders(Aves, 2017) Ozdek, Ali; Keseroglu, Kemal; Er, Serap; Unsal, Selim; Gunduz, MehmetObjective: To investigate the correlation of ocular vestibular evoked myogenic potential (o-VEMP) and video head impulse (v-HIT) test results with caloric test results in peripheral vestibular disorders, and to define an algorithm for investigating vestibular disorders. Methods: All patients underwent an ENT physical examination, an MRI scan, videonystagmography, an o-VEMP test, and a v-HIT test. The patients who had central-type vertigo, PBBV, and vestibular migraine were excluded from the study. Overall, 30 patients were included in the study. Results: Canal paralysis was observed in 22 patients (73.3%). Pathological o-VEMP and v-HIT test results were obtained in 19 (63.3%) and 8 (26.7%) patients, respectively. Pathological o-VEMP or v-HIT was observed in 17 of 22 (77.3%) patients with canal paralysis. Sensitivity and specificity of a combination of o-VEMP and v-HIT test results for detecting canal paralysis were 77.3% and 80.1%, respectively. Conclusion: It does not seem that o-VEMP and v-HIT tests can replace the caloric test to detect vestibular hypofunction. However, these two tests can be used as first-line tests to initiate vestibular evaluation. Because their voices are their instruments to teach, they use it frequently. That's why our teachers are under great risk of having voice problems and diseases The purpose of this work, finding a meaningful contribution on the improvement of the quality of teachers' voices by comparison before and 4 weeks after the completion of the voice hygiene education to them. The teachers are selected from public schools and private courses within the province of Ankara. The voice hygiene education was given to 32 teachers in total: 10 female and 6 male teachers who work full time (6-12 hours) and 10 female and 6 male teachers who work part time (1-4 hours). As a control group, the voice hygiene education was not given to 38 teachers in total: 8 female and 12 male teachers who work full time (6-12 hours) and 10 female and 8male teachers who work part time (1-4 hours). The complete experiment group consists of 70 people. As a subjective evaluation method, voice handicap index and survey is used. Electroglottography (EGG) which is used in acoustic analysis was preferred as an objective test material, Measuring teachers' voices and using MDUP analysis program, Fo, % jitter, % shimmer and MFZ parameters were used as data. Every voice sample for every subject was analyzed by computer program. To evaluate the effects of voice hygiene course, UHI was filled before, during and after the 4-weeks voice hygiene education. The grading of UHI results we used are min.13 and max.65. The evaluated UHI results were 13 - 39 points. After giving hygiene education, the average UHI point was decreased by 5.22 points compared to UHI values before giving hygiene education (p < 0.000). The significant decrease of total points after giving hygiene education was observed. Positive emotional, psychological and social effects occurred on teachers. It can be said that the approximate MFZ values of subject group after giving hygiene education are higher than 1.69 compared to before giving hygiene values (p = 0.000) to evaluate the vocal performance on MFZ tests. The significantly high MFZ values are thought to be related with voice hygiene education. In our study, there is no significant difference observed in Fo, % jitter and % shimmer values, due to the perturbation parameters before and after hygiene (p > 0.05). When the mean values are observed from the parameters, it is seen that there were minimal decrements. The decrease in these values gives rise to thought that the voice hygiene course affects the healing side positively. After giving voice hygiene education, the observed decrease in these values are thought to be related with closeness of voice cords, tension and the decrease in voice quality caused by reduced tension. The voice hygiene education given to teachers is observed to be helpful. It is thought to reduce the probability of having bad use of voice as a permanent habit and helpful about early diagnosis and treatment. Methods: A total of 25 elders between ages 65-84 years (74.3 +/- 5.4) and 25 individuals between ages 18-45 years (30.4 +/- 4.2) were recruited for the study. After ear, nose, and throat examination, ipsilateral and contralateral acoustic reflex thresholds at 500, 1000, 2000, and 4000 Hertz (Hz) were determined and a reflex decay test at contralateral 500 Hz was conducted. Ipsilateral acoustic reflex thresholds were obtained with high-frequency band, low-frequency band, and wide band noise, and the results were compared with ipsilateral acoustic reflexes at 500, 1000, 2000, and 4000 Hz. Results: There was no statistically significant difference between the two groups in ipsilateral and contralateral acoustic reflex measurements at 500, 1000, 2000, and 4000 Hz (p>0.05). Negative reflex decay was obtained in all participants and no statistically significant difference between the two groups was observed in terms of reflex decay thresholds (p>0.05). Acoustic reflex with high-frequency band noise was observed in five of nine elders whose acoustic reflexes were not obtained at 2000 and 4000 Hz, whereas acoustic reflex with low-frequency band noise was observed in one of six elders who did not show reflexes at 500 and 1000 Hz. Conclusion: It was concluded that although some changes were observed due to age, middle ear and stapes muscles work normally in geriatric group. In the reflex decay test, reliable results were obtained at contralateral 500 Hz. Acoustic reflex measurements with low-and high-frequency band noise may also be used to assess middle ear functions.Öğe Examination of Insert Ear Interaural Attenuation (IA) Values in Audiological Evaluations(Canadian Soc Clinical Investigation, 2016) Gumus, Nebi M.; Gumus, Merve; Unsal, Selim; Yuksel, Mustafa; Gunduz, MehmetPurpose: The purpose of this study was to evaluate Interaural Attenuation (IA) in frequency base in the insert earphones that are used in audiological assessments. Methods: Thirty healthy subjects between 18-65 years of age (14 female and 16 male) participated in our study. Otoscopic examination was performed on all participants. Audiological evaluations were performed using the Interacoustics AC40 clinical audiometer and ER-3A insert earphones. IA value was calculated by subtracting good ear bone conduction hearing thresholds of the worst airway hearing threshold. Results: In our measuring for 0.125-8.0 kHz frequency were performed in our audiometry device separately for each frequency. IA amount in the results we found in 1000 Hz and below frequencies about 75-110 dB range avarage is 89 +/- 5dB, in above 1000 Hz frequencies in 50-95 dB range and avarage it is changed to 69 +/- 5dB. Conclusion: According to the obtained findings the quantity of melting in the transition between the ears are increasing with the insert earphones. The insert earphone should be beside supraaural earphone that is routinely used in clinics. Difficult masking applications due to the increase in the value of IA can be easily done with insert earphones.Öğe Functional analysis of ESM1 by siRNA knockdown in primary and metastatic head and neck cancer cells(Wiley, 2018) Bender, Onur; Gunduz, Mehmet; Cigdem, Sadik; Hatipoglu, Omer Faruk; Acar, Muradiye; Kaya, Mesut; Grenman, ReidarBackgroundGenetic factors play a large role in cancer, and thus, there is a great desire to understand the effects of different genes in cancer and to also develop gene therapy for better treatments. Therefore, the development of alternative diagnosis and therapy modalities is of utmost importance. The aim of our study was to illuminate the role of ESM1 (endothelial cell-specific molecule-1, also known as Endocan) in proliferation and migration of head and neck cancer, thus helping to pave the way for new treatment modalities and predictive biomarkers. MethodsESM1 expression was shown with immunofluorescence assay using confocal laser scanning microscope in primary and metastatic head and neck cancer cells. ESM1 expression was knocked down by RNA interference in head and neck cancer cells. Knockdown efficiency was evaluated by quantitative real-time RT-PCR and Western blot. Cell proliferation and migration assays were performed by xCELLigence real-time cell analysis system. ResultsImmunofluorescence assay showed nuclear localization and high expression of ESM1 in primary and metastatic head and neck cancer cells. ESM1 mRNA and protein levels were significantly decreased in ESM1-knockdown cells compared to control. ESM1-knockdown cells showed reduced proliferation and migration activity when compared to control cells. Kulak burun boğaz muayenesisonrası, 500, 1000, 2000 ve 4000 Hertz'de (Hz) ipsilateralve kontrlateral akustik refleks eşikleri saptandı vekontralateral 500 Hz'de refleks erime testi gerçekleştirildi.İpsilateral olarak yüksek frekans, alçak frekans vegeniş bandlı gürültü ile akustik refleks eşiklerine bakıldıve ipsilateral 500, 1000, 2000 ve 4000 Hz akustikreflekslerle karşılaştırıldı.Bulgular: İpsi ve kontralateral akustik refleks ölçümlerinde500, 1000, 2000 ve 4000 Hz' de gruplar arasındaistatistiksel olarak anlamlı fark bulunamadı (p>0.05).Tüm katılımcılarda refleks erime negatif olarak eldeedildi ve refleks erime eşik karşılaştırmasında gruplararasında anlamlı bir fark bulunamadı (p>0.05). 2000ve 4000 Hz'de beraber ipsilateral akustik refleks eldeedilemeyen dokuz yaşlının beşinde yüksek frekansbandlı gürültü ile aksutik refleks elde edilirken, 500ve 1000 Hz'de beraber akustik refleks elde edilemeyenaltı yaşlının sadece birinde alçak frekans bandlı gürültüile akustik refleks elde edildi.Sonuç: Geriatrik grupta yaşa bağlı kulakta bazı de-ğişimler olsa da orta kulak ve stapes kasının normalçalıştığı sonucuna varıldı. Refleks erime testinde isekontralateral 500 Hz ile güvenilir sonuçlar elde edilebildi.Alçak ve yüksek frekans bandlı gürültü ileakustik refleks ölçümleri de orta kulak fonksiyonlarınıdeğerlendirmede kullanılabilir.Öğe Giant Cell Reparative granuloma in maxilla and mandibula: A case report(2014) AKDERE, Elif; KURTARAN, Hanif; ARK, Nebil; UĞUR, Kadriye Şerife; YÜKSEL, ALPER; SONGÜR, Timur; Gunduz, MehmetDev hücreli reperatif granülom, etyolojisi tam olarak bilinmeyen, baş-boyun bölgesinde sıklıkla mandibula ve maksillayı tutan, benign karakterli ancak bazen lokal agresifseyir gösteren lezyonlardır. Tutulum bölgesinde genelliklekitle oluşturarak ve kemik yapıda deformite yaparak belirtiverirler. Tedavisinde farklı medikal ve cerrahi yöntemlerınuygulanması ve nadir görülmesi lezyonu ilginç kılmaktadır.Biz bu olgu sunumunda sol alt çene ve ağız içinde 8 aydır bulunan ağrılı şişlik, şikayetiyle başvuran, 47 yaşında bayanhastayı sunduk. Medikal tedaviye rağmen şikayetlerininartması üzerine ameliyat edilen hastanın patoloji incelemesi dev hücreli reperatif granulom olarak rapor edildi. Ayırıcıtanı ve tedavi de ki gelişmeler de tartışıldı.Öğe Giant Cell Reparative Granuloma in the Maxilla and Mandibula(Galenos Yayincilik, 2014) Akdere, Elif; Kurtaran, Hanifi; Ark, Nebil; Ugur, Kadriye Serife; Yuksel, Alper; Songur, Timur; Gunduz, MehmetGiant cell reparative granuloma is a disease with an unknown etiology, characterized of benign locally aggressive lesions invading mandible and maxilla in the head and neck region. These lesions usually present as a mass and cause deformities at bony structures. The use of various medical and surgical procedures in the treatment of the disease with its rare occurrence makes these lesions interesting. In this case report we present a 47 years old female patient who had a painful swelling in the left lower left jaw and oral cavity since 8 months. After her complaints proceeded despite medical therapy, she has been operated and pathology report confirmed a giant cell reparative granuloma. We also discuss differential diagnosis and the achievements in treatment procedures.Öğe Investigation of the effects of 5-fluorouracil and thymoquinone on head and neck cancer stem cells(Canadian Soc Clinical Investigation, 2015) Acar, Muradiye; Gunduz, Mehmet; Ocak, Tarik; Gurses, Hacer Esra; Cetin, Elif Nihan; Ceylan, Furkan Sacit; Sefa, AlperenPurpose: A subpopulation of cancer stem cells (CSCs) in head and neck squamous cell carcinoma (HNSCC) play a critical role in invasion, metastasis, and tumour recurrence post-therapy. 5-Fluorouracil (5-FU) is a pyrimidine analogue that is used in the treatment of HNSCC. Thymoquinone (TQ), found in Cumin (Nigella sativa) seed oil, has anti-diabetic, anti-oxidant, anti-inflammatory, and antitumour effects. The purpose of this study was to examine the effects of 5-FU and TQ on CSCs. Methods: ALDH1 (+) cells were isolated from the HNSCC cell line UT-SCC-74A using magnetic activated cell sorting (MACS) and sphere formation was confirmed. Real-time RTPCR revealed was used to measure expression levels of a number of stem cell markers, including OCT4, SOX2, and KLF-4. To investigate the effects of the therapeutic agents, 5-FU and TQ, concentration-dependent assay procedures were used and cell viability after treatment was measured using XTT assay. Results: ALDH1 (+) cells formed spheres while the ALDH1 (-) cells did not. Real-time RT-PCR revealed that a number of stem cell markers, including OCT4, SOX2, and KLF-4, were up-regulated in the CSCs. XTT results showed that, when given alone at a dose of 50 ng/mL, neither agent had a statistically significant cytotoxic effect on CSCs. However, when dosed in combination, TQ and 5-FU had a synergistic effect on CSC cytotoxicity. Conclusion: We report the effects of TQ and 5-FU on CSCs in HNSCC.Öğe Investigation of the expression of RIF1 gene on head and neck, pancreatic and brain cancer and cancer stem cells(Canadian Soc Clinical Investigation, 2016) Cila, Hacer E. The aim of this study was to compare RIF1 gene expression in head and neck, pancreatic cancer and glioma cell lines and the cancer stem cells isolated from these cell lines. Methods: UT-SCC-74 from Turku University and UT-SCC-74B primary tumor metastasis and neck cancer cell lines, YKG-1 glioma cancer cell line from RIKEN, pancreatic cancer cell lines and ASPC-1 cells from ATCC were grown in cell culture. To isolate cancer stem cells, ALDH-1 for UT-SCC-74 and UT-SCC-74B cell line, CD-133 for YKG-1 cell line and CD-24 for ASPC-1 cell line, were used as markers of cancer stem cells. RNA isolation was performed for both cancer lines and cancer stem cells. RNAs were converted to cDNA. RIF1 gene expression was performed by qRT-PCR analysis. RIF1 gene expression was compared with cancer cell lines and cancer stem cells isolated from these cell lines. The possible effect of RIF1 gene was evaluated. Results: In the pancreatic cells, RIF1 gene expression in the stem cell-positive cell line was 256 time that seen in the stem cell-negative cell line. Conclusion: Considering the importance of RIF1 in NHEJ and of NHEJ in pancreatic cancer, RIF1 may be one of the genes that plays an important role in the diagnoses and therapeutic treatment of pancreatic cancer. The results of head and neck and brain cancers are inconclusive and further studies are required to elucidate the connection between RIF1 gene and these other types of cancers.Öğe Long-term results of one staged multilevel surgery with tongue suspension surgery or one level palatal surgery for treatment of moderate and severe obstructive sleep apnea(Springer, 2016) Yuksel, Alper; Ugur, Kadriye Serife; Kizilbulut, Gultekin; Ark, Nebil; Kurtaran, Hanifi; Kaya, Mesut; Gunduz, MehmetThe objective of this study is to evaluate and compare the long-term efficacy of the one staged multilevel surgery (MLS) with tongue suspension (TBS) surgery or one level palatal surgery for treatment of moderate and severe obstructive sleep apnea (OSA). This is a prospective cross-sectional study. Setting: University hospital. Thirty-three patients diagnosed as moderate to severe OSA. Patients, with a parts per thousand yen50 % retropalopharyngeal obstruction on the Muller maneuver, were treated with palatal surgeries (PS) and patients, with a parts per thousand yen50 % retropalopharyngeal obstruction on the Muller maneuver with a parts per thousand yen50 % base of the tongue collapse, were treated with palatal surgeries and tongue suspension surgery (TBS). Patients were evaluated with one night polysomnography before the surgery and 24 months after the surgery. Patients completed Epworth sleepiness scale (ESS), snoring VAS (visual analog scale) before and 24 months after the surgery. Nine-teen patients with a mean age of 46.1 +/- A 8.3 underwent palatal surgeries (PS) and 14 patients with a mean age of 41.4 +/- A 8.9 underwent PS plus TBS. Success rate in TBS+PS group was 57.1 % and in PS group was 42.1 %. In both groups total apnea-hypopnea index (AHI) values significantly decreased after 2 years (p < 0.025) but there was no statistically significant difference between TBS+PS and PS groups. Supine AHI levels were reduced statistically significant in both groups postoperatively (p < 0.025). There was not any significant difference postoperatively in non-supine AHI levels in both groups (p > 0.025). There were significant postoperative changes in ODI, AVO2, MOS, ESS, Snoring VAS values in PS group (p < 0.025). In TBS+PS group there was a significant difference postoperatively only in ODI values. Treatment of OSA patients with retropalatal or retropalatal and retroglossal obstruction, in a one staged surgery, is a safe and easy procedure. Materials and Methods: This study was conducted on 21 Wistar albino rats in four groups. Cisplatin, cisplatin plus NAC, NAC alone, and saline were intraperitoneally administered. The rats were anesthetized to study the otoacoustic emissions before and after the experiment. Results: The OAEs were attenuated in the cisplatin group: (-1.31/-4.90), (6.28/4.58), (4.00/0.93), (2.73/-3.74), (2.66/-4.53). The group that received NAC in addition to cisplatin had better OAEs. Conclusion: Cisplatin ototoxicity can be detected by OAE testing in rats, and NAC may reduce the ototoxic effect.
