Öğe Aile Hekimlerine Yönelik Kulak Burun Boğaz Muayenesi ve İşitme Değerlendirme Eğitiminin Etkinliğinin Değerlendirilmesi(Duzce University, 2024) Tetik, Burcu Kayhan; Gündüz, Emrah; Yaşar, Şeyma; Kayhan, MehmetAmaç: Biz çalışmamızda birinci basamak sağlık kuruluşlarında çalışan aile hekimlerinin KBB hastalıkları ile ilgili bilgi düzeylerini araştırmayı, sıklıkla başvuran KBB hastalarını nasıl yönettiklerini incelemeyi, muayene sırasında yaşadıkları sorunları tespit edip verilen eğitimin etkisini incelemeyi amaçladık. Metod: Katılımcılara öncelikle araştırmacı tarafından literatür baz alınarak oluşturulmuş 30 soruluk bir ön test uygulandı. Ardından araştırmacılardan KBB uzmanı tarafından 2 gün süren ve günlük 1.5 saatlik toplu bir eğitim verildi ve son test uygulandı. Daha sonra araştırmacıların en çok hata yaptıkları veya yorumsuz kaldıkları konular kaydedildi ve bu konuları içeren bir eğitim programı oluşturularak eğitim verildi. Bulgular: Çalışmaya alınan 30 yaş üzeri 21 aile hekimlerinden 2 (%14.29)’si erkek 18 (%85.71)’i kadındır. Çalışmadaki aile hekimlerinin tıp fakültesinden mezun olma süresi incelendiğinde, 5-10 yıl aralığında olanların sayısı 4 (%19.05), 10-20 yıl aralığında olanların sayısı 10 (%47.62), 20 yıldan fazla bir zamana sahip olanların sayısı ise 7 (%33.33)’dir. Çalışmaya katılan aile hekimlerinin tamamı birinci basamak hekimliği için KBB eğitiminin önemli olduğunu düşünmektedir. Sonuç: KBB hastalıkları birinci basamakta sık görülen hastalıklardandır. Aile hekimlerinin daha rahat hastalarını yönetebilmeleri için mezuniyet sonrası dönemde KBB hastalıkları alanında rotasyonlarının uygun olacağı kanaatindeyiz.Öğe Coblation cryptolysis method in treatment of tonsil caseum-induced halitosis(Elsevier, 2021) Erdur, Ömer; Çelik, Turgut; Fırat Koca, Çiğdem; Yaşar, ŞeymaPurpose: Halitosis, is a social problem affecting many patients seeking help from clinicians. Tonsil stones can cause halitosis and especially occur in crypts of palatine tonsils. Coblation cryptolysis is an alternative method for tonsil caseum treatment. The coblation technology includes passing a radiofrequency bipolar electrical current through a medium of normal saline which results in the production of a plasma field of sodium ions. In this study, our aim was to investigate the effectiveness of coblator cryptolysis treatment method in chronic caseous tonsillitis-induced halitosis. Methods: We included in our study 28 patients who underwent coblator cryptolysis surgery for halitosis due to chronic caseous tonsillitis. The efficacy of treatment and the presence of caseoma were evaluated with the Finkelstein test, organoleptic test and VAS before the procedure and at the 6th month control after the treatment was completed. Results: At the 6th month follow-up after the procedure (a single coblation cryptolysis) we found that 23 of the patients (82.1%) had no caseum. There was a statistically significant change in Finkelstein measurements before and after the procedure (p < 0.001). Organoleptic measurements demonstrated that 21 patients had no halitosis postoperatively and the mean organoleptic test score was calculated as 0.39 ± 0.79 after the procedure. The recovery was statistically significant (p < 0.001). The mean VAS score before coblation cryptolysis was 8.0 ± 1.33 (range 5-10). On the other hand 6 months after a single coblation cryptolysis session, the mean VAS score was 1.25 ± 1.78 (range: 0-6). This difference was statistically significant (p < 0.001). Conclusions: Our results suggest that coblation crptolysis is an effective, safe, minimally invasive and practical alternative method in treatment of halitosis due to tonsil caseums. We did not observe any complication after the procedure.Öğe Covid 19 Hastalarında Kalış Süresini ve Mortaliteyi Etkileyen Risk Faktörleri: Laboratuvar Parametreleri, Komorbiditeler ve Demografik Özellikler(Duzce University, 2021) Delen, Leman Acun; Erdoğan, Esra; Yaşar, ŞeymaAmaç: Covid 19, ölümcül pnömoniye ve ciddi komplikasyonlara neden olabilen güncel pandemik hastalıktır. Hastalığın farklı evrelerinde farklı biyokimyasal parametrelerin düzeyinde artış görülmekte ve bu parametreler hastalığın seyri konusunda önemli bilgiler vermektedir. Bu çalışmada, Covid 19 hastalarındaki yatış süresi ve mortalite oranı ile biyokimyasal parametreler arasındaki ilişkinin araştırılması amaçlanmıştır. Materyal ve Metot: Bu retrospektif çalışmaya, hastanemizde yatan toplam767 Covid 19 hastası dahil edildi. Hastanenin veri tabanı ve hasta dosyaları incelenerek hastaların demografik özellikleri, yatış süreleri, ek hastalıları ve biyokimyasal parametreler taranarak kaydedildi. Hastalar yatış gününe göre, 1. Grup: 7 gün ve altı, 2. Grup: 8-10 gün arası 3. Grup: 11-13 gün arası, 4. Grup: 14 gün ve üzeri olmak üzere toplam 4 gruba ayrıldı. Hastaların demografik özellikleri, laboratuvar bulguları, ek hastalıkları ve mortalite oranları bu gruplara göre düzenlenerek istatiksel analiz yapıldı. Bulgular: Hastaların 459 (60%)’u erkek 308 (40%)’i kadındı. Grup 4 hastalarındaki CRP düzeyi, grup 1’deki hastalara göre istatiksel anlamlı olarak yüksekti (p=0.002). Grup 4 hastalarındaki LDH, PRO_BNP ve prokalsitonin düzeyi hem grup 1 hemde grup 2’ye göre istatiksel anlamlı olarak yüksekti (sırasıyla p<0.001, p=0.026, p=0.007). Fibrinojen düzeyi, grup 4 deki hastalarda grup 2’deki hastalara göre anlamlı olarak yüksekti (p=0.011). DM, HT, LDH ve ferritin düzeyi yüksekliği ölüm riskinin anlamlı olarak artırmıştı (sırasıyla p=0.022, p=0,006 p<0.001, p=0,041). Benzer olarak yaş ve yatış gününün uzaması ölüm riskini anlamlı olarak artırmıştı (p<0.001). Sonuç: Çalışmamızın sonuçları, Covid 19 hastalarının yatış süresinin uzamasıyla CRP, LDH, PCT, PROBNP ve fibrinojen düzeyinin doğru orantılı olarak arttığını ve hastalığın şiddetliye ilişkili olabileceğini göstermektedir. Yine bu sonuçlar, LDH ve ferritin düzeyinin, yaş, yatış gününün uzaması, HT, DM gibi ek hastalıkların varlığının mortalite oranını artırdığını ve prognoz açısından daha spesifik parametreler olduğunu göstermektedir.Öğe Delta Neutrophil Index as a Diagnostic Marker of Neonatal Sepsis(IOS Press, 2021) Melekoğlu, Nuriye Aslı; Yaşar, Şeyma; Keskin, MehmetObjective Sepsis diagnosis is challenging due to nonspecific symptomatology in newborns. Timely diagnosis is essential for reducing sepsis-related morbidity and mortality. This study was performed to determine the diagnostic value of the delta neutrophil index (DNI) for detection of neonatal sepsis and to compare its efficacy with other conventional markers. Methods This study was conducted at a tertiary hospital in newborns with confirmed sepsis (n = 59), suspected sepsis (n = 46), and in age- and weight-matched controls (n = 49). DNI, white blood cell count, C-reactive protein (CRP) level, and platelet measurements were determined, and blood cultures were performed at the onset of symptoms. Results The mean DNI was significantly higher in confirmed and clinical sepsis groups compared with the control group. (6.9 ± 9.3, 1.9 ± 2.1, and 0.4 ± 0.5, respectively, p < 0.001). ROC curve analysis also showed that the combination of DNI and CRP had the highest sensitivity (86%), specificity (100%), and positive predictive value (100%) for predicting neonatal sepsis. DNI values were significantly higher in nonsurvivors (p < 0.05). Conclusion DNI could be used as a reliable diagnostic marker for neonatal sepsis, and high DNI could predict sepsis development and unfavorable outcomes. The diagnostic capability of DNI may be increased by assessing CRP measurements simultaneously.Öğe Peripheral facial paralysis during the COVID-19 pandemic(İnönü Üniversitesi, 2022) Aydın, Şükrü; Fırat Koca, Çiğdem; Çelik, Turgut; Kelleş, Mehmet; Yaşar, ŞeymaThe mechanism surrounding idiopathic peripheral facial nerve paralysis remains unclear, though viral infections and even immunizations have been suspected of its origin. Thus, the relationship between COVID-19 and facial paralysis should be studied. With this research, we aimed to investigate the characteristics of facial paralysis during the COVID-19 illness as well as the relationship between facial paralysis and COVID-19, the length of time needed for recovery, concurrence with COVID-19 infection, and whether facial paralysis is a late complication or initial symptom of the disease. Forty-five patients thought to have had idiopathic peripheral facial paralysis were included in the study. Pure tone audiometry, COVID-19 PCR tests, and contrast-enhanced ear MRIs were performed on all participants. A standard prednisolone treatment protocol was followed. Participants were monitored for one month; we recorded whether they had COVID-19 previously, initially, or contracted it within the one-month testing period. At the same time, facial paralysis recovery rates were recorded and used in statistical analyses. PCR test at initial admission was reported as positive for COVID-19 in only one participant (2.2%). We discovered an improvement delay regarding facial paralysis in participants who had had COVID-19 previously (p<0.001). Prednisolone therapy used for peripheral facial paralysis did not pose an additional risk for COVID-19. Having had COVID-19 previously may cause delayed recovery of peripheral facial paralysis. Peripheral facial paralysis may be both a late manifestation as well as an early symptom of COVID-19.
