    Galectin-1 as a potential diagnostic biomarker in polycystic ovary syndrome

    OBJECTIVE: This study was aimed at comparing the routine laboratory pa-rameters and Galectin-1 levels of control and polycystic ovarian syndrome patients.PATIENTS AND METHODS: 88 patients di-agnosed with polycystic ovary syndrome and 88 healthy controls were considered for the study. Age groups of the patients ranged from 18 to 40. Serum TSH, Beta HCG, glucose, insulin, HO-MA-IR, Hb1A1c, triglyceride, total cholesterol, LDL FSH, LH, E2, prolactin, testosterone, SHBG, DHESO4, HDL, Gal-1 levels were analyzed for each subject.RESULTS: FSH, LH, LH/FSH, E2, prolactin, testosterone, SHBG, DHESO4, HDL and Gal -1 values of the subjects included in the study were statistically significantly different between the groups (p<0.05). Gal-1 and DHESO4 showed a strong positive connection (p=0.05). The sen-sitivity of Gal-1 level in PCOS patients was cal-culated as 0.997 and specificity as 0.716.CONCLUSIONS: High levels of Gal-1 in PCOS patients suggest that it increases due to overex-pression in response to inflammation.

    Basal blood concentrations of some orexigenic and anorexigenic hormones in obese and nonobese individuals according to blood groups

    OBJECTIVE: Obesity is a serious public health problem associated with excessive food intake. Regulation of food intake in highly organized organisms is under the control of a large number of orexigenic and anorexigenic molecules. Therefore, the main purpose of this study has been to determine the relationship between obesity and some of the circulating orexigenic and anorexigenic peptides that have a role in appetite control and to determine whether the concentrations of these molecules differ according to blood groups. PATIENTS AND METHODS: The study included 400 individuals of whom 100 were obese women, 100 obese men, 100 healthy men and 100 healthy women. Obese women and men were divided into 4 groups, according to their blood groups. In the control group, healthy women and healthy men were similarly divided into 4 blood groups. Each blood group within the groups, therefore, had 25 participants. RESULTS: When leptin, nesfatin-1, obestatin and neuropeptide-Y, ghrelin and galanin levels of the control group and obese participants were compared, regardless of blood groups, leptin, nesfatin-1, obestatin and neuropeptide-Y were significantly higher, whereas only the ghrelin levels were significantly lower in obese patients. When the amounts of these hormones were measured according to gender, the situation was similar. When leptin, nesfatin-1, obestatin and neuropeptide-Y values of the control and obese participants' blood groups were compared with each other; these hormones were high in all blood groups; however, leptin levels in A blood group, nesfatin-1 levels in AB and O blood group, obestatin levels in AB blood group, neuropeptide-Y levels in A, B, AB blood groups were significantly higher. When the ghrelin levels of the blood groups in the control group and obese participants were compared, it was only significantly lower in the AB blood group. The ghrelin levels in the other blood groups of the obese individuals were again low, but not significantly so. When the distribution of hormones according to gender was evaluated, a situation parallel to the above results was recorded. CONCLUSIONS: Leptin, nesfatin-1, obestatin and neuropeptide-Y and galanin levels of obese individuals were significantly higher than the control values, whereas the ghrelin values were significantly lower regardless of blood groups. Also. these hormones in blood partly varied with ABO blood groups. These different concentrations of hormones in ABO blood groups might be related with stimulation or suppression of appetite in human. However. further studies in other ethnic groups are needed to confirm these results.

    Clinical features and predisposing factors of delirium due to COVID-19 pneumonia in intensive care units

    OBJECTIVE: Delirium is an acute disorder in which attention, perception, memory, thought, mood, psychomotor activity and sleep-wake cycles change rapidly. Delirium is also a common clinical syndrome in patients hospitalized in intensive care units due to COVID-19 pneumonia. We reviewed clinical features and predisposing factors of delirium according to psychomotor subtype in patients hospitalized in the intensive care units due to COVID-19 pneumonia. PATIENTS AND METHODS: 64 patients who were hospitalized in the intensive care units due to COVID-19 pneumonia were included. Delirium status and psychomotor subtype were determined by applying the Confusion Assessment Method for the Intensive Care Unit scale to the patients daily. The gender, age, comorbidity, treatments, intubation, and mortality rates of the patients were recorded. Multivariate analyses were performed by examining predisposing factors. arterial blood gases, hemograms, biochemistry, and brain magnetic resonance imaging. RESULTS: There were 64 patients in delirium clinic, 65.6% (n=42) of them were male. Hypokinetic delirium was more common in 60.9% (n=39). 79.4% of the patients who received ventilator support were male (p=0.013). When mortality was analyzed in this group, hypoactive delirium was found to be significantly higher (p=0.035). In addition, leukocyte levels were higher in patients with hypokinetic delirium (p=0.029). Ferritin and fibrinogen levels were higher in patients with hyperkinetic delirium (p=0.039, p=0.008, respectively). CONCLUSIONS: The presence of additional diseases such as advanced age, male gender, hypertension, coronary artery disease, dementia, and hypoxia were factors that increased the frequency of delirium. In addition, the mortality rate was higher in patients with hypokinetic delirium.

    Asprosin, visfatin and subfatin as new biomarkers of obesity and metabolic syndrome

    OBJECTIVE: Metabolic syndrome (MetS) and obesity are important public health problems associated with adipose tissue mass. Asprosin, visfatin, and subfatin are new members of which fate in MetS and obesity has not been fully revealed yet. Thus, this study was to investigate the association between asprosin, visfatin, subfatin. and biochemical values, demographic data, and body composition measurement values in MetS patients with and without obesity. PATIENTS AND METHODS: Blood samples were taken from a total of 90 people, including 31 MetS patients with obesity. 29 MetS patients without obesity. and 30 healthy (control). Asprosin, visfatin. and subfatin were studied by the ELISA method. RESULTS: There was a negative correlation between asprosin and Body Mass Index (BMI) in the MetS + Obese group. The correlations between asprosin and urea and fasting insulin (FI) levels in the MetS group were positive and statistically significant (p < 0.05). While there was a statistically significant negative correlation (p < 0.05) between visfatin and BMI in the MetS + Obese group. the correlation with waist circumference in the MetS + Obese and MetS groups was statistically significant and negative (p < 0.05). There was a statistically significant negative relationship (p < 0.05) between aspartate aminotransferase value and visfatin. The results between visfatin values and asprosin and subfatin in all groups were significant (p < 0.05). CONCLUSIONS: There is a direct relationship between circulating amounts of asprosin, visfatin, and subfatin hormones and age, weight, height, diastolic blood pressure, high-density lipoprotein-cholesterol, aspartate aminotransferase. alanine transaminase, and creatinine. Therefore, asprosin, visfatin, and subfatin hormones are the new biomarkers of metabolic turbulence.

    The plasma galectin-3 level has high specificity and sensitivity for predicting postoperative atrial fibrillation after coronary artery bypass surgery

    OBJECTIVE: Postoperative new -on-set atrial fibrillation (POAF) commonly occurs after coronary artery bypass graft (CABG) surgery. This study aimed to determine the utility of the preop- erative netrin-1 and galectin-3 levels for predict- ing POAF following CABG surgery, as well as that of postoperative serial measurement for assessing these markers' patterns of expression.PATIENTS AND METHODS: This prospec- tive cohort study included 50 patients that un- derwent CABG surgery. The plasma netrin-1 and galectin-3 levels were measured via en- zyme-linked immunosorbent assay (ELISA) be- fore surgery (baseline) and at 6, 12, and 24 h af- ter surgery. The patients were divided into two groups according to the occurrence of POAF; the POAF (+) group and the POAF (-) group. RESULTS: In total, 26 patients developed POAF, whereas 24 remained in sinus rhythm. Base- line galectin-3 levels were higher in the POAF (+) group than in the POAF (-) group (30.7 +/- 10.1 pg mL-1 and 15.7 +/- 3.6 pg mL-1, respectively). The post-CABG surgery galectin-3 level increased in both the POAF (+) and POAF (-) groups at 6 h (46.2 +/- 26.3 pg mL-1 and 24.9 +/- 5.9 pg mL-1, respective- ly), 12 h (45.2 +/- 24.1 pg mL-1 and 26.6 +/- 9.3 pg mL-1, respectively), and 24 h (54.2 +/- 33.5 pg mL-1and 28.6 +/- 7.7 pg mL-1, respectively). The plasma ne- trin-1 level did not differ between groups at base- line or at 6, 12, and 24 h post CABG surgery.CONCLUSIONS: Whereas netrin-1 does not appear to have any utility as a marker for the de- velopment of POAF in CABG surgery patients, the plasma galectin-3 level has high specifici- ty and sensitivity for predicting POAF following CABG surgery and could be considered a mark- er for predicting POAF.

    Evaluation of the effect of CPAP treatment on voice in obstructive sleep apnea with objective and subjective voice analyzes

    OBJECTIVE: Obstructive sleep ap-nea (OSA) is a multifactorial disease that is the most common among sleep-related respirato-ry disorders. In our study, we aimed to compare the objective and subjective voice analysis re-sults of the patients with CPAP treatment indi-cation after polysomnography performed in the sleep disorders center before the start of CPAP treatment, the 1st month, and the 3rd month after the treatment.PATIENTS AND METHODS: Patients were asked to say the vowels /alpha:/ for 5 seconds, re-spectively for voice recording. MPT and S/Z ra-tios were also recorded. Using the Praat voice analysis program Jitter%, Shimmer%, HNR, f0 values were obtained. VHI-10 questionnaire was applied. The voice analysis results of the patients before and after the treatment were compared.RESULTS: 26 patients were included in the study. Since 8 of these patients did not come to the 3rd month evaluation, the study was complet-ed with the data of 18 patients. In our study, at the end of the 3rd month, no difference was found in terms of the fundamental frequency, Shimmer%, HNR, and S/Z values. There was a statistically significant decrease in Jitter% at the 1st month after treatment and 3rd month after treatment compared to pre-treatment (p=0.05, p=0.018). There was a statistically significant decrease in MPT at the 1st month after treatment and 3rd month after treatment compared to pre-treatment (p<0.001, p<0.001). There was a statistically sig-nificant decrease in VHI-10 at the 1st month and 3rd month after treatment compared to pre-treat-ment. Furthermore, there was statistically signifi-cant difference between 3rd month after treatment compared to 1st month after treatment. (p=0.043, p=0.030, p=0.029).CONCLUSIONS: Our study showed that CPAP treatment had both objective and subjective pos-itive effects on voice recovery in the OSA patient group.

    Can evaluating adrenal glands in computed tomography contribute to predicting the prognosis of hospitalized COVID-19 patients?

    OBJECTIVE: The novel coronavirus disease 2019 (COVID-19) may affect the adrenal glands. Therefore, it is important to evaluate the morphologic appearance of the adrenal glands by thorax computed tomography (CT). On CT scans, stranding in peripheral fatty tissue with enlarged adrenal glands may indicate signs of adrenal infarction (SAI). The present study aimed to evaluate the incidence of SAI and determine whether this finding may contribute to predictions of the prognosis of COVID-19. PATIENTS AND METHODS: A total of 343 patients who had been hospitalized at Malatya Training and Research Hospital between September 1 and 30, 2020, with a diagnosis of COVID-19 were enrolled in this study. All patients underwent thorax CT scans that included their adrenal glands. RESULTS: Of the enrolled patients, 16.0% had SAI. Moreover, 41.8% of patients with SAI and 15.3% of patients without SAI were treated in the Intensive Care Unit (ICU). Patients with SAI had a significantly higher rate of ICU admission (p < 0.001). Mortality rates were also significantly higher among patients with SAI than those without p < 0.001). CONCLUSIONS: In this study, it was found that COVID-19 patients with SAI may have a poorer prognosis. More comprehensive studies are needed on this subject, but the present study may provide helpful preliminary information in terms of prognosis.

    Cerebrovascular radiological features of COVID-19 positive patients

    OBJECTIVE: To investigate acute cerebrovascular diseases (stroke and intracranial hemorrhage) by cranial radiologic examinations of patients infected with coronavirus disease 2019 (COVID-19) and with neurological signs. PATIENTS AND METHODS: Between March 2020 and May 2021, patients who were admitted to the Emergency Department and had a positive reverse transcription-polymerase chain reaction (RT-PCR) test and underwent Multidetector Computed Tomography (MDCT) and/or Magnetic Resonance Images (MRI), and/or diffusion MRI due to neurological findings were included in the study. RESULTS: The study reviewed a total of 925 patients, including 404 (43.67%) female and 521 (56.32%) male patients. The distribution of imaging methods was as follows: 805 (71%) patients had cranial MDCT, 71 (6.35%) patients had MRI, and 241 (21.57%) patients had diffusion MRI. Of the total 925 patients, 128 (13.8%) patients were detected with cerebrovascular diseases, 92 (9.9%) patients were detected with ischemic or hemorrhagic stroke, 37 (4%) patients were detected with intraparenchymal hemorrhage, 10 (1.1%) patients were detected with subarachnoid hemorrhage, and four (0.43%) patients were detected with subdural hemorrhage. There was no statistically significant difference in the incidence of subdural, subarachnoid, parenchymal hemorrhage. and stroke in terms of gender. While there was a significant difference in stroke according to age, there was no statistically significant difference in subdural, subarachnoid, and parenchymal hemorrhagic. Three (0.32%) patients were diagnosed with acute disseminated encephalomyelitis (ADEM)'s-like demyelinating lesions. CONCLUSIONS: Cerebrovascular diseases, which may cause severe disability and even threaten the patient's life, should be kept in mind, especially in COVID-19 patients who present with neurological symptoms.

    A comparison of two surgical methods in the treatment of shoulder adhesive capsulitis

    OBJECTIVE: The purpose of the study was to compare the outcomes of arthroscopic capsular release surgery and manipulation of patients with resistant primary adhesive capsulitis (AC) under anesthesia.PATIENTS AND METHODS: The study comprised forty-four patients who had surgery after being given a diagnosis of primary AC. Patients who had both passive and active glenohumeral and scapulothoracic movements equal to or less than 100 degrees elevation and less than 50% of external rotation compared to the contralateral side were considered to have resistant adhesive capsuli-tis and were included in the study. Conservative treatments such as intra-articular steroid injec-tions and physical therapy had failed to relieve the pain in these patients for at least six months. The patients who took part in the trial underwent manipulation under anesthesia (group 1) and arthroscopic capsular release (group 2) opera-tions. The chosen surgical procedure was cho-sen at random and based on the surgeon's Pref-erences. Examining the patients' demographic information. After treatment, the patients were examined at three-month, six-month, and one-year intervals. Joint range of motion, visual an-alogue scale (VAS), and Constant-Murley shoul-der scores were all recorded. Statistics were used to compare the outcomes of the two surgi -cal techniques in this study both before and af -ter the procedure. RESULTS: The study's participants' gender, side, extra procedure, and age factors did not show a statistically significant difference be-tween groups 1 and 2 (p < 0.05). According to the age, gender, side, additional process, and ho-mogeneous distribution throughout the groups. No statistically significant difference was dis-covered between groups 1 and 2 in any of the measurements taken from study participants during the follow-up period: Pre-op visual pain scores (VPS), Post-op 3rd month VPS, Post-op 1st year VPS, Pre-op Constant score, Post-op 6th month Constant score, and Post-op 1st year Constant score (p < 0.05). The change in VPS and Constant Score values over time did not show a statistically significant difference between the groups (p < 0.05). A statistically significant difference between the groups was discovered in each of the Pre-op period and Post-op 6th month VPS assessments (p < 0.05).CONCLUSIONS: Although there was no statistically significant difference between the two studied therapies, the surgical method was shown to be more beneficial in both groups the shorter the pre-op period was between the onset of the complaints and the operation.

    Is the use of Tenofovir Dipivoxil fumarate effective and safe in preventing vertical transmission in pregnant women with chronic HBV with high viral load?

    OBJECTIVE: In our country, transmission from mother to baby is the most common form of transmission of viral hepatitis B. A high viral load in the mother and HBeAg positivity pose the greatest risk of transmission from mother to baby. The best way to pre-vent this is to try to eliminate the viral load in the mother by using a strong antiviral such as prenatal TDF in mothers with a high viral load during pregnancy. This study aimed to evaluate the efficacy and safety of TDF in pregnant women with high viral load.PATIENTS AND METHODS: Seventy patients with hepatitis B e-antigen positive and negative were included in the retrospective study conducted in our clinic. In 35 cases, pregnant women with HBeAg (+) positive chronic HBV and HBV-DNA levels of 107 copies/mL were be-tween 18 and 27 weeks of pregnancy. The preg-nant women took 300 mg of TDF per day. There were 35 untreated HBeAg-negative, chronic HBV patients in the control group. Babies born to HBeAg-positive and HBeAg-negative mothers are given an initial dose of 200 IU of hepatitis B immune globulin (HBIG) and 20 g of recombi-nant hepatitis B vaccine in the first 12 hours af-ter birth, followed by 4, 8, and 24 weeks. HBsAg and HBV-DNA findings were examined in new-born serum 28 weeks after birth.RESULTS: Postpartum 28 weeks, none of the babies born to HBeAg-positive mothers treated with TDF had HBsAg positivity, while 3.5% of babies born to HBeAg-negative mothers and not treated with TDF had HBsAg positivity and immunoprophylaxis failure. There was no statistically significant difference between the treatment and control groups regarding maternal height, weight, gestational age, or congenital malformations (p<0.05). There was no signifi-cant difference between the side effects seen in mothers. In the examination performed at the 28th week postpartum, a statistically signifi-cant decrease in HBV-DNA levels was observed in mothers who received TDF treatment com-pared to those who did not (88.5%) (p<0.05). In 31 of the 35 patients receiving TDF treatment, ALT was reported to be normalized in 25 of the 35 patients who did not receive TDF treatment (p<0.05).CONCLUSIONS: It has been observed that the use of TDF, which has a strong efficacy and high barrier, in the second and/or third trimester of pregnancy reduces transmission rates without causing side effects in both the mother and the newborn, thereby preventing vertical transmission of viral hepatitis B from the mother to child.

    The relationship between food addiction and emotional eating in individuals at risk for anorexia nervosa

    OBJECTIVE: The aim of this study was to determine the relationship between anorexia nervosa, food addiction, and emotional eating.SUBJECTS AND METHODS: The study was carried out with 395 university students. The Eating Attitude Test (EAT)-26 was used to determine the risk of anorexia nervosa in individuals. Yale Eating Addiction Scale (YFAS) and The Dutch Eating Behavior Questionnaire (DEBQ) were used to determine the eating behaviors that may be effective in anorexia nervosa. Digital scales were used for weight measurement, and a non-stretchable tape measure according to standard techniques was used for measuring height, waist, and hip circumferences.RESULTS: In this study, in which individuals at (high and medium) risk of anorexia nervosa were examined, 62.03% were females, and the mean age was 25.21 +/- 4.33 years. The mean BMI value was 22.25 +/- 3.48 kg/cm2. In the study, 8.35% of people with both anorexia nervosa (AN) risk and food addiction made up 19.75% of the food addict population (p=0.023). The distribution of DEBQ scores by sub-dimension shows that AN risk decreases as external eating score decreases. The group at the highest risk for AN also had the highest emotional eating score (p=0.029). In the multiple linear regression analyses, emotional eating behavior, gender, and BMI were predictive factors for eating attitude or anorexia nervosa risk.CONCLUSIONS: Food addiction increases the risk of eating disorders and the most important factor affecting them is BMI. Women were more likely than men to have anorexia nervosa and food addiction. This study informed young adults about food addiction, eating disorders, and anorexia nervosa.

    Tibial graft fixation in anterior cruciate ligament reconstruction: multiple tibial tunnel technique (collateral tunnel technique)

    - OBJECTIVE: This study aimed to compare the outcomes of patients with an anterior cruciate ligament (ACL) rupture who underwent tibial fixation using the multiple tibi-al tunnel fixation (MTTF) and standard tibial fix-ation methods.PATIENTS AND METHODS: This retrospec-tive study was conducted between January 1, 2020, and August 1, 2021. MTTT was applied to 43 patients diagnosed with ACL rupture. Of the 43 patients who met the study criteria, 38 were classified as Group 1. In the clinic where the study was conducted, 40 of 57 patients who underwent standard ACL reconstruction by opening a single tibial tunnel were assigned to Group 2. The Endobutton technique was used for fixation of the graft to the femur in both groups. Bioabsorbable and postfix screws were used for the tibial fixation of the patients in Group 2. For patients in Group 1, a bioab-sorbable screw, a postfix screw, and an addi-tional MTTT fixation were performed for tibial fixation. Lachman, anterior drawer, Pivot-Shift test results, Lysholm and IKDC knee evalua-tion scores of the patients in both groups were compared.RESULTS: In this study, there was no sig-nificant difference between the groups for the anterior drawer, Lachman, and Pivot-Shift test results at the final control (p > 0.05). There was a significant difference between the two groups for the Lysholm and IKDC scores at the final controls (p < 0.05). There was a significant dif-ference in the Lysholm and IKDC scores be-tween the groups (p < 0.05).CONCLUSIONS: In conclusion, ACL recon-struction was performed using the MTTF tech-nique in this study. Due to the additional fixa-tion, it was observed that the patients had a more successful knee function after the surgery.

    Dosimetric analysis of SIB whole brain radiotherapy planning: Comparison of coplanar VMAT and tomotherapy techniques

    Background Radiotherapy techniques have advanced significantly over the past few decades. Whole-brain radiotherapy combined with a simultaneous integrated boost (WBRT+SIB) is increasingly used to treat limited brain metastases. Purpose To retrospectively compare helical tomotherapy and coplanar volumetric modulated arc therapy (VMAT) for WBRT with a SIB-WBRT in patients with multiple brain metastases. Additionally, it emphasizes the importance of selecting appropriate evaluation indices when comparing SIB plans. Materials and methods Fifteen patients with 2- 3 metastatic lesions were retrospectively analyzed in this study. Treatment planning was performed using TomoHD and eclipse planning systems for tomotherapy and VMAT, respectively. Dose-volume histograms were used to assess the doses delivered to the target volumes and organs at risk (OARs). Quantitative metrics, including the homogeneity index (HI), conformity index (CI), and plan quality index (PQI), were used for the evaluation. Results Tomotherapy yielded significantly higher D98% values for both the planning target volume (PTV) WB and PTV_met compared with VMAT (p < 0.05). It also provided lower D-max and D-mean values for the lenses and eyes (p < 0.001 and p < 0.02, respectively). Tomotherapy was superior in terms of PTV whole-brain CI and PTV_met HI and CI (p < 0.05). However, no significant difference was observed in the PQI values between the techniques (p > 0.05). Conclusion Both tomotherapy and VMAT achieved acceptable target volumes and OAR doses in SIB applications. Tomotherapy showed advantages in terms of dose conformity and critical organ sparing.     Delineating the Adult Phenotype of PGM2L1-Related Neurodevelopmental Disorder

    PGM2L1 is a crucial enzyme exhibiting glucose 1,6-bisphosphate synthase activity, with predominant expression in brain tissue. In 2021, biallelic pathogenic variants in the PGM2L1 gene were first linked to a neurodevelopmental disorder characterized primarily by developmental delay in four pediatric cases. In this study, we aimed to delineate the adult phenotype associated with the PGM2L1-related neurodevelopmental disorder and to perform functional characterization of the identified variant. Two siblings presenting with neurodevelopmental delay were evaluated clinically and genetically. Exome sequencing of the older sibling revealed a homozygous nonsense variant, c.277C>T p.(Gln73Ter), in the PGM2L1 gene. This variant results in truncation leading to loss of key functional domains including the substrate binding site, catalytic active site, and protein stability regions. Quantitative analysis demonstrated a significant reduction in PGM2L1 gene expression in both siblings compared to controls (p value < 0.01). Unlike previously reported pediatric cases, the second sibling exhibited additional features including scoliosis, renal anomaly, tooth loss, hypothyroidism, bladder trabeculation, anhidrosis, and temperature intolerance, notably in the absence of obesity. These cases represent the first detailed description of an adult phenotype associated with a biallelic pathogenic variant in PGM2L1, expanding the clinical spectrum of this neurodevelopmental disorder.

    Effect of Acceptance and Commitment Therapy on Motivation and Adherence to Immunosuppressive Medication in Liver Transplant Recipients: A Randomized Controlled Trial

    Background: After liver transplantation, recipients continue to receive immunosuppressive drug regimens at varying doses for years to ensure graft function and survival. Although liver transplant recipients are aware of the importance of immunosuppressive drug regimens, treatment compliance may decrease over time due to low motivation, putting recipients at risk of organ rejection. Acceptance and commitment therapy (ACT) appears to be an ideal solution for increasing motivation and, indirectly, treatment compliance. Objective: The aim of this study was to investigate the effect of ACT on adherence to immunosuppressive medication and motivation in liver transplant recipients. Methods: This study was conducted at an organ transplant hospital with the participation of recipients who had completed at least 6 months post-liver transplant. The sample included n = 140 liver transplant recipients (n = 70 experimental, n = 70 control). The study was randomized and controlled. The control group received no intervention, while the experimental group underwent an eight-session ACT program administered by a psychologist. Data collection utilized a personal characteristics form, the Adherence to Immunosuppressive Drug Regimen Scale (AIDRS), the Mindfulness Scale (MS), and the Adult Motivation Scale (AMS). Data analysis was performed using descriptive statistical methods and tests of significance for differences between variables. Findings: According to the homogeneity tests of this study, there was no statistically significant difference between the control and experimental groups in terms of individual characteristics (age, time since transplantation, immunosuppressive drugs, comorbidities, etc.). In the experimental group, the level of adherence to the immunosuppressive drug regimen after ACT was very close to the maximum AIDRS score. In the control group, the level of compliance with the immunosuppressive drug regimen was close to the mean value of the AIDRS. The differences mentioned in both groups were statistically significant (p < 0.05). In addition, the scores obtained from the MS and YMS were higher than those in the pretest. When the experimental and control groups were compared, the experimental group's final test YMS score was almost twice that of the control group, and this difference was statistically significant (p < 0.05). Conclusion: The importance of full compliance with the immunosuppressive drug regimen in improving graft function after liver transplantation is well known. The results of this study showed that liver transplant recipients who underwent ACT demonstrated higher adherence to the immunosuppressive drug regimen due to increased motivation and conscious awareness. We recommend the use of ACT to increase adherence to the immunosuppressive medication due to its effectiveness.

    Molecular Extremes of Ovarian Response: Divergent PI3K/AKT/mTOR Signaling and Oxidative Stress Patterns in Low and High Responders Undergoing In Vitro Fertilization

    Ovarian response extremes are associated with distinct alterations in the follicular microenvironment, yet the underlying molecular mechanisms remain incompletely understood. This study investigated oxidative stress, antioxidant responses, apoptosis, and PI3K/AKT/mTOR signaling in granulosa cells from women undergoing intracytoplasmic sperm injection, stratified into poor (POR), normal (NOR), and high ovarian response (HOR) groups (n = 30). Follicular fluid oxidative stress markers, antioxidant systems, and regulatory factors were analyzed, while intracellular reactive oxygen species, DNA fragmentation, and chromatin integrity were evaluated in granulosa cells. In addition, PI3K/AKT/mTOR pathway activity was assessed using immunohistochemistry and immunofluorescence. Compared with NOR, both POR and HOR groups exhibited increased oxidative stress, reflected by elevated MDA levels (p < 0.05), accompanied by divergent antioxidant responses, including reduced GSH levels in POR and a progressive increase in SOD activity from NOR to HOR (p < 0.05). Although melatonin and TGF-beta levels did not differ significantly, melatonin showed a trend toward lower levels in POR and higher levels in HOR. Granulosa cell ROS production and apoptosis-related markers were significantly increased in both response extremes, particularly in POR (p < 0.05). Consistently, PI3K/AKT/mTOR signaling displayed differential regulation, with reduced activation in POR and enhanced activation in HOR compared to NOR (p < 0.05). Overall, NOR appears to reflect a balanced redox and molecular state, whereas both poor and high responses are characterized by oxidative stress, cellular damage, and dysregulated PI3K/AKT/mTOR signaling. These findings suggest that deviation from physiological equilibrium, rather than response magnitude alone, is a key determinant of granulosa cell dysfunction and impaired follicular competence.

    The evaluation of the training, the approach to epilepsy and epileptic seizure, which was given to teachers in Turkiye

    Introduction: Epilepsy is the most common neurological disorder among humans after headaches. According to the World Health Organization, approximately 50-65 million individuals were diagnosed with epilepsy throughout the world, and around two million new cases of epilepsy are added to this figure every year. Methods: Designed as descriptive and cross-sectional research, this study was performed on 132 elementary school teachers. Training on epilepsy and epileptic seizure was given to teachers. The pretest and posttest research data were collected with the face-to-face interview method. In this process, the epilepsy knowledge scale was used as well as a survey form that had questions designed to find out about teachers' personal characteristics. The Statistical Package for Social Science 25.0 was utilized in the statistical analysis of research data. In the research, the statistical significance was identified if the p-value was below.05 (p < .05). Results: Of all teachers participating in the study, 59.1% were female, 90.2% were married, and 47.7% witnessed an epilepsy seizure before. The mean of teachers' pretest epilepsy knowledge scores was 8.43 +/- 4.31 points before the training while the mean of their posttest epilepsy knowledge scores was 12.65 +/- 2.48 points after the training. The difference between the means of pretest and posttest scores was statistically significant (p = .000). After the training, there was a statistically significant increase in means of scores obtained by teachers from each item of the epilepsy knowledge scale (p < .05). Conclusions: As there was a statistically significant improvement in levels of teachers' knowledge about both epilepsy and epileptic seizure after the training, it is recommended that the training about the approach to epilepsy and epileptic seizure be given to all teachers, and additionally, including these topics in the course curricula of universities is recommended. image

    A novel TCNN-Bi-LSTM deep learning model for predicting sentiments of tweets about COVID-19 vaccines

    Many researchers in various disciplines have focused on extracting meaningful information from social media platforms in recent years. Identification of behaviors and emotions from user posts is examined under the heading of sentiment analysis (SA) studies using the natural language processing (NLP) techniques. In this study, a novel TCNN-Bi-LSTM model using the two-stage convolutional neural network (TCNN) and bidirectional long short-term memory (Bi-LSTM) architectures was proposed. While TCNN layers enable the extraction of strong local features, the output of these layers feeds the Bi-LSTM model that remembers forward-looking information and capture long-term dependencies. In this study, first, preprocessing steps were applied to the raw dataset. Thus, strong features were extracted from the obtained quality dataset using the FastText word embedding technique that pre-trained with location-based and sub-word information features. The experimental results of the proposed method are promising compared to the baseline deep learning and machine learning models. Also, experimental results show that while the FastText data embedding technique achieves the best performance compared to other word embedding techniques in all deep learning classification models, it has not had the same outstanding success in machine learning models. This study aims to investigate the sentiments of tweets about the COVID-19 vaccines and comments on these tweets among Twitter users by using the power of Twitter data. A new dataset collected from Twitter was constructed to be used in experimental results. This study will facilitate detecting inappropriate, incomplete, and erroneous information about vaccination. The results of this study will enable society to broaden its perspective on the administered vaccines. It can also assist the government and healthcare agencies in planning and implementing the vaccination's promotion on time to achieve the herd immunity provided by the vaccination.

    Effects of Positive Psychotherapy-Based Education on Pain, Psychological Resilience, and Mental Well-Being After Lumbar Canal Stenosis Surgery: A Randomized Controlled Trial

    Aim This study was conducted to investigate the effects of positive psychotherapy-based education on pain, psychological resilience, and mental well-being after lumbar canal stenosis surgery.Materials and Methods A randomized controlled experimental study was conducted with 66 participants (33 control, 33 experimental) at the Brain and Neurosurgery Department of Medical Center X between March 2025 and July 2025. The control group received no intervention, while the experimental group received 8 weeks of psychoeducation based on positive psychotherapy. Pretests were administered to the control and experimental groups in March 2025. The training of the experimental group was administered between April 2025 and May 2025. Data collection tools included the Descriptive Characteristics Form, Pain Scale, Brief Resilience Scale, and Mental Well-Being Scale. It was determined that the data showed normal distribution, and parametric tests were used in the analyses. Chi-square and Fisher's exact tests were used to compare the descriptive characteristics of the patients in the experimental and control groups. A t-test was applied to the dependent and independent groups to determine the effect of the training.Findings In the study, no statistically significant difference was found in the comparison of the pretest pain, psychological resilience, and mental well-being scales of the control and experimental groups (p = 0.259, p = 0.177, p = 0.428). In the study, a statistically significant difference was found in the comparison of the posttest pain, psychological resilience, and mental well-being scales of the control and experimental groups (p = 0.001). In the study, a statistically significant difference was found in the pretest-posttest pain, psychological resilience and mental well-being scale total score averages of the experimental group (p = 0.001).Conclusion and Recommendations Positive psychotherapy-based education was effective in reducing pain and increasing psychological resilience, and mental well-being in patients after lumbar spinal stenosis surgery. Based on these findings, positive psychotherapy-based education can be recommended as an adjunct to pharmacological treatment for not only physical but also psychological recovery.

    Quercetin Exhibits Nephroprotective Properties Against Tartrazine-Induced Nephrotic Injury: Effects on Oxidative Stress, Kidney Function, Inflammation, Renal Tissue Morphology, and Apoptotic Pathway

    We investigated first time in the literature the effects of tartrazine, a common industrial dye, and quercetin, a possible protective, on the kidneys. The rats were randomly assigned to the control, tartrazine, quercetin, and tartrazine + quercetin groups, with each group consisting of eight Wistar albino rats. The trials lasted for 1 month, after which kidney tissues and blood samples were collected. In the tartrazine group, increases were observed in malondialdehyde (MDA), superoxide dismutase (SOD), total oxidant status (TOS), oxidative stress index (OSI), tumor necrosis factor alpha (TNF-alpha), interleukin-6 (IL-6), glomerular diameter and damage, histopathological damage score, glomerular and tubular caspase-3 immunoreactivity H-score, as well as serum urea, uric acid, and creatinine levels in the kidney tissue. Additionally, kidney tissue histopathology and apoptotic deteriorated in the same group. The deteriorated biochemical and histopathological parameters improved with quercetin administration. Tartrazine led to nephrotoxicity in rats, as indicated by kidney tissue oxidant capacity, inflammation, apoptosis, increased kidney function tests, and deterioration in histopathology. Quercetin exhibited strong antioxidant, antiapoptotic, and anti-inflammation activity and can be used as a protective agent against tartrazine-induced nephrotoxicity.
