Hashimoto tiroiditinde ınterlökin-35 düzeyinin değerlendirilmesi

Özet
Amaç: Hashimoto tiroiditi kronik otoimmün bir tiroidit olup dünyada hipotroidinin en sık nedenidir. Son dönemlerde HT'nin patofizylojisine yönelik yapılan hayvan çalışmaları ve in vitro çalışmalarda otoimmün tiroid hastalığında CD4+CD25+Foxp3+ hücre düzeyinin dolaşımda ve tiroid dokusunda yetersiz olduğu ya da fonksiyonun azaldığı gösterilmiştir. Treg hücreleri antiinflamatuvar etkili interlökin-10 ve interlökin-35 sekrete etmektedir ve immünolojik self toleransta önemli rol oyanamaktadır. Hashimoto tiroiditinde IL-35 düzeyi ile ilgili insan çalışması yapılmamıştır. Bizim bu çalışmada amacımız Hashimoto tiroiditi ile IL-35 düzeyi arasındaki ilişkiyi ortaya koymak ve bunu normal populasyon ile karşılaştırmaktır. Material ve metod: Çalışmaya 166 kişi dahil edildi. Kontrol grubu, ötiroid, subklinik ve aşikar Hashimoto tiroiditi olmak üzere 4 gruba ayırdık. IL-35 düzeyi, serum örneklerinde elisa yöntemiyle çalışıldı. Sonuçlar: Gruplar arasında IL-35 düzeyleri açısından istatiksel olarak anlamlı farklılık saptanmadı (p=0,285). Kontrol grubu ile aşikar hipotiroidili hasta grubu arasında IL-35 düzeyi açısından istatiksel olarak anlamlı farklılık saptanmıştır (p=0,046). TSH değerini 10 alıp, <10 uIU/ml ve ?10 uIU/ml olmak üzere iki yeni grup oluşturduğumuzda ise IL-35 düzeyi açısından istatiksel olarak anlamlı farklılık saptandı (p=0,024). Bu veri IL-35 düzeyinin azalmasının, Hashimoto tiroiditi patogenezinin oluşmasında anahtar bir role sahip olabileceğini desteklemektedir. Tartışma: IL 35 düzeyi ile HT arasında bir ilişki söz konusudur. Bu ilişkinin daha net bir şekilde ortaya konulabilmesi için kontrol edilebilir faktörlerin etkisinin minimalize edildiği daha geniş hasta katılımlı çalışmalara ihtiyaç vardır. Background and aims: Hashimoto's thyroiditis a chronic autoimmune thyroiditis and it is the most common reason of hypothyroidism in the world. Recently evidence suggested that the influence of pathogenesis of Hashimoto's thyroiditis in the thyroid tissue and circulating levels of CD4+CD25+Foxp3+cells (treg cells) are shown to be inadequate or decreased function in autoimmune thyroid disease. Treg cells expressed of interleukin-10 and interleukin-35 as anti-inflammatory effect.Treg cells have an important role in of immunological self-tolerance. Nowadays there is no human studies associated the relationship between IL-35 and Hashimoto's thyroiditis. We aim to investigate the relationship between the level of IL-35 and Hashimoto's thyroiditis, and its compareness with the normal population. Material and methods: We enrolled 166 peoples in to this study. We classfied in to four differntial groups; as group one, healthy control group; group two,euthyroid Hashimoto's thyroiditis; group three, subclinic Hashimoto's thyroiditis; group four overt Hashimoto's thyroiditis. IL-35 levels were measured by ELISA in serum samples. Results: There was no difference significantly among groups according to IL-35 levels (p=0,285).However we demonstrated that, patients with overt hypothyroidism has lower level of IL-35 than healthy controls (p=0,046). We groupped the patients as the cutt of TSH level is 10, group one is TSH<10 uIU/ml, group two is TSH?10 uIU/ml. Group two has decreesed IL-35 level compared as group one (p=0,024). This data suggested that decreesed IL-35 level may play a key role in the development and the pathogenesis of Hashimoto's thyroiditis. Conclusion: We explaned the relationship between the level of IL-35 and Hashimoto's thyroiditis, the prospective studies contains as the larger studies population and homogeneity of demographics characteristics.
